| [目的]探究抑郁症的中医病机本质，为抑郁症的中医临床诊疗提供新思路。[方法]搜索相关文献，并运用分析、对比、总结等方法进行文献研究，结合历代医家的抑郁症临床经验，逐步推导出胆虚肝郁中医理论的合理性，最后附上医案一则予以验证。[结果]以心悸胆怯、遇事善惊等为主要表现的胆虚之人容易肝郁，胆虚证与抑郁症的发生、发展和预后关系密切。胆虚是指胆汁虚少导致的胆的不充盈状态，而胆腑充盈是“胆主决断”的生理基础。医家通过用健脾益气、养肝补血、补肾益精等治疗方法保持胆汁生化有源，以及通过对痰、火、瘀血等实邪的清除以保持胆汁的藏而不泻，进而恢复胆主决断的功能是治疗抑郁症的基本思路。所附验案外寒内热、胆虚焦虑、表里俱实，治以解表通里、清热解毒，方用防风通圣散加减，取得较好疗效。[结论]从胆虚出发论治抑郁症不仅能丰富中医学对抑郁症发病机理的认识，也能为抑郁症的中医辨治提供一个新思路。 |

| [Objective] To explore the essence of the TCM pathogenesis of depression， and provide new train of thought for the traditional Chinese medicine clinical diagnosis and treatment of depression.[Methods] We planned to search related literature， using methods such as literature research， comparative analysis combined with past dynasties doctor’s clinical experience in the depression， gradually derived empty of gallbladder liver depression, the rationality of the Chinese medicine theory.[Results] With palpitations cowardly, easily surprised as the main performance of empty of gallbladder people， The empty of gallbladder is closely associated with the development and prognosis of depression. Empty of gallbladder refers to the less gallbladder bile virtual， whereas bile relieving filling is the physiological basis of physicians to make decisions. By replenishing the spleen and Qi, nourishing the liver blood and invigorating the the bile generation， and through the removal of the blood stasis phlegm， fire， the real evil to keep the bile hidden， and to restore the function of the gallbladder for main determination is the basic way of thinking.[Conclusion] Treatment of depression from empty of gallbladder can not only enrich Chinese medicine understanding of the pathogenesis of depression， but also provide a new way of thinking for depression syndrome differentiation of traditional Chinese medicine. |
