| [目的]总结裘昌林老师诊疗重症肌无力合并白癜风的临床经验。[方法]通过复习相关中医文献、跟师学习、整理医案等方式，总结分析裘昌林老师在治疗重症肌无力合并白癜风的病因病机、治疗思路、用药特色及中西医结合辨证论治方面的学术观点和临床经验，并列举具体医案予以验证。[结果]裘师认为，重症肌无力合并白癜风并不少见，皆与自身免疫相关。白癜风可分虚实，但重症肌无力者，久病体虚，两者合并必为虚证，治疗注重健脾益气与养血结合。所举医案，贯彻裘师辨证施治经验，遣方用药取益气健脾为君，补益肝肾为臣，养血祛风通络为佐使，重用黄芪、白术、党参、制黄精、仙灵脾益气健脾补肾，同时选用白芷、女贞子、旱莲草、补骨脂、鸡血藤养血祛风补肾针对白癜风治疗，获得满意疗效。[结论]裘昌林老师对重症肌无力合并白癜风的病因病机、辨证论治和用药的临床经验具有系统化的认识，遣方用药从虚论治随证加减，疗效满意，副反应少，充分说明中医药治疗重症肌无力合并白癜风的优势，值得传承推广。 |

| [Objective]To summarize the clinical experience of QIU Changlin in treating myasthenia gravis with vitiligo.[Methods]By reviewing relevant literature of traditional Chinese medicine, studying in clinic with teacher and tidying medical records,summarize and analyze QIU Changlin's academic views and clinical experience in the treatment of myasthenia gravis combined with vitiligo in terms of etiology, pathogenesis, treatment ideas, drug characteristics and syndrome differentiation and treatment of the combination of Chinese and western medicine.And enumerate specific medical cases to verify.[Results]Teacher QIU thinks that myasthenia gravis combined with vitiligo is not uncommon and is associated with autoimmunity.Vitiligo can be divided into deficiency and excess, while myasthenia gravis is of mostly deficiency syndrome with its long course of disease. The combination of the two will be of deficiency syndrome. The treatment focuses on the tonifying the spleen and Qi and nourishing blood. According to the medical case, the experience of QIU's syndrome differentiation and treatment was carried out, sending prescription to take medicine to benefit Qi and invigorate spleen for the king,tonifying liver and kidney for the minister,raising blood with Qufeng Tongluo for the assistant envoy.Reuse of astragalus, atractylodes macrocephala, codonopsis pilosula, rhizoma polygonatum, epimedium, invigorating spleen and kidney.At the same time, angelica dahurica, ligustrum lucidum, eclipta, psoralea and caulis spatholobi were selected to treat vitiligo.And the satisfactory curative effect was obtained.[Conclusion]Teacher QIU Changlin has a systematic understanding of the etiology,pathogenesis,syndrome differentiation, treatment and clinical experience of medication of myasthenia gravis with vitiligo.On the treatment of deficiency with syndrome addition and subtraction, this has satisfactory efficacy and little side effects.This fully demonstrates the advantages of traditional Chinese medicine in the treatment of myasthenia gravis combined with vitiligo, which is worthy of inheritance and promotion. |
