      [目的]综述藤梨根从“痰”论治恶性肿瘤的进展,为藤梨根在临床上用于肿瘤治疗提供新思路。[方法]在Pubmed、中国知网(China National Knowledge Infrastructure,CNKI)等数据库中,以“藤梨根”“恶性肿瘤”“痰”为中文检索词,以“Radix Actinidiae Chinensis”“malignant tumor”“phlegm”为英文检索词,检索2000年至2019年的相关文献。通过对相关文献进行整理分析,归纳总结藤梨根从“痰”论治恶性肿瘤的理论基础、临床应用及实验研究结果。[结果]藤梨根具有健脾利湿、化痰散结、活血消肿、清热解毒等功效,目前在临床上广泛用于恶性肿瘤的治疗。藤梨根可通过下调B细胞淋巴瘤/白血病-2(B-cell lymphoma/leukemia-2,Bcl-2)、核转录因子-κB p65(nuclear factor-κB p65,NF-κB p65)以及基质金属蛋白酶-9(matrix metalloproteinase-9,MMP-9)蛋白表达,上调半胱氨酸蛋白酶-3(cysteine protease-3,Caspase-3)蛋白表达,抑制磷脂酰肌醇3-激酶/蛋白激酶B(phosphoinositide3-kinase/protein kinase B,PI3K/Akt)信号通路激活等机制,抑制肿瘤细胞增殖、诱导细胞凋亡、激发细胞失巢凋亡等。痰饮微环境中的痰浊是体内津液代谢异常的主要产物,其与肿瘤微环境中的炎症细胞、炎症因子和趋化因子等较为相似,而Bcl-2、Caspase-3、NF-κB p65、MMP-9等蛋白及PI3K/Akt通路均与痰饮微环境有关。[结论]藤梨根从“痰”论治恶性肿瘤具有理论基础、临床应用及实验研究依据。藤梨根在宏观上可痰、瘀、毒同治,在微观上可改善痰饮微环境,进而发挥抗肿瘤作用。

      [Objective] To explore the progress of Radix Actinidiae Chinensis in the treatment of malignant tumor from the perspective of phlegm and provide new ideas of clinical use of Radix Actinidiae chinensis in the treatment of malignant tumors.[Methods] By searching the databases of Pubmed, China National Knowledge Infrastructure(CNKI) and others with the Keywords of “Radix Actinidiae Chinensis”“malignant tumor” and “phlegm”, related literatures from 2000 to 2019 were retrieved and analyzed, and the theoretical basis, clinical application and experimental research basis of Radix Actinidiae Chinensis in the treatment of malignant tumors from the perspective of phlegm were summarized.[Results] Radix Actinidiae Chinensis has the functions of invigorating the spleen and eliminating dampness, dissipating phlegm and resolving masses, promoting blood circulation and removing swelling, clearing heat and toxin. It is widely used in the treatment of malignant tumors. Radix Actinidiae Chinensis can play a role in inhibiting tumor cell proliferation, inducing apoptosis, and stimulating anoikis by down-regulating B-cell lymphoma/leukemia-2(Bcl-2), nuclear factor-κB p65(NF-κB p65) and matrix metalloproteinase-9(MMP-9) protein expression, up-regulating cysteine protease-3(Caspase-3) protein expression, and inhibiting phosphoinositide-3-kinase/protein kinase B(PI3K/Akt) signaling pathway activation. The phlegm microenvironment has similar properties to the tumor microenvironment. Phlegm turbidity in the phlegm microenvironment is the main product of abnormal body fluid metabolism, which is very similar to inflammatory cells, inflammatory factors and chemokines in the tumor microenvironment. Bcl-2, Caspase-3, NF-κB p65 and MMP-9 protein, and PI3K/Akt pathway are related to phlegm microenvironment.[Conclusion] Radix Actinidiae Chinensis has theoretical basis, clinical application and experimental research basis for the treatment of malignant tumors from the perspective of phlegm. It can play an anti-tumor role by treating phlegm, blood stasis and heat toxin at the same time at the macro level, and improve the phlegm microenvironment at the micro level.
