| [目的]总结全国老中医药专家学术经验指导老师、浙江省名中医牟重临主任运用补中益气汤加减治疗腹部恶性肿瘤术后并发症的临床经验。[方法]通过随师侍诊，选取腹部恶性肿瘤术后肠梗阻、疮口不愈合、肢体活动障碍、失眠、食欲不振、发热等六则医案，从病机、症候、用药特色等方面论述牟主任运用补中益气汤加减的临床经验。[结果]牟主任认为肿瘤成因究其根本是正气不足，尤其是腹部恶性肿瘤手术更是损伤人体脾胃正气，从而导致较多并发症。本文选取牟主任临诊治疗的六则医案，究其共同病机均是脾胃损伤，中气虚陷，而致诸邪杂气，乘虚而袭，故治以补中益气汤加减，从补益中焦脾胃气血入手，均获得显著的临床疗效。[结论]牟主任应用补中益气汤辨治腹部恶性肿瘤术后并发症的经验，为腹部恶性肿瘤术后并发症的治疗提供了临床思路，值得学习和借鉴。 |

| [Objective] To summarize the experience of Director MOU Chonglin， the academic experience instructor of the national old Chinese medicine expert and famous Chinese medicine doctor in Zhejiang Province， in the treatment of postoperative complications of abdominal malignancies with Buzhong Yiqi Decoction. [Methods] Through follow-up with the teacher， six medical records were selected， including postoperative intestinal obstruction， wound unhealing， limb movement disorder， insomnia， loss of appetite and fever， the clinical experience of Director MOU in using modified Buzhong Yiqi Decoction was discussed from the aspects of pathogenesis， symptoms and medication characteristics.[Results] Director MOU Chonglin believes that the basic cause of malignancies is deficiency of vital energy， especially the operation of abdominal malignancies damages the vital Qi of the spleen and stomach， and causes many complications. In this paper， six cases treated by Director MOU were selected， and the common pathogenesis was injury of the spleen and stomach， deficiency and collapse of Qi of tri-Jiao， resulting in attacking of all kinds of evil and miscellaneous Qi. Therefore， the modified Buzhong Yiqi Decoction was adopted， which could obtain significant clinical effect， starting from tonifying the Qi and blood of tri-Jiao.[Conclusion] Director MOU Chonglin‘s experience of treating abdominal malignancies with syndrome differentiation and treatment of Buzhong Yiqi Decoction provides clinical ideas for the treatment of postoperative complications of abdominal malignancies， which is worth learning and reference. |
