| [目的] 总结介绍杨骏教授针灸治疗三叉神经痛的临床经验。[方法] 通过跟师学习、记录医案、查阅相关书籍与文献的方式，分析总结杨师治疗三叉神经痛的中西医学术观点、选穴特色及临床经验，并列举典型医案以佐证。[结果] 杨师认为无论外因内因皆使面部经络失阻，或久病经络失养而诱发此病，以不通则痛、不荣则痛为关键病机，提出了“行气血，调虚实，通经络”的治疗原则。选穴上中西互参，将现代神经解剖与针灸理论相结合，精选腧穴，强调针刺手法及针刺方向，同时调神以移痛；注重局部与整体的结合，选取太冲、合谷、中脘等穴。治疗上采用多种综合疗法，上关、下关两穴电针加强针感，下关穴选取温针灸以增强镇痛效果，针刺结束后留置腕踝针以减少发作频数，疗效确切。所举医案中患者辨为气虚血瘀证，治疗上气血双补、活血化瘀，使头面部经络通畅，患者痛症自除。[结论] 杨骏教授针刺治疗三叉神经痛，结合中西医发病机制，化繁为简，选穴上中西互参，注重调神，整体施治；治法上注重杂合以治，极大地减轻患者疼痛症状，效果颇佳，值得临床推广。 |

| [Objective] To summarize and introduce Professor YANG Jun‘s clinical experience in treating trigeminal neuralgia with acupuncture. [Methods] By learning from teachers， recording medical cases and consulting relevant books and literatures， Professor YANG‘s academic viewpoints of traditional Chinese medicine and western medicine， acupoint selection characteristics and clinical experience in treating trigeminal neuralgia were analyzed and summarized， and one typical medical case was listed as evidence. [Results] Professor YANG believes that both external and internal causes lead to the loss of the facial meridians or the loss of the meridians after a long period of illness causes the disease， and the key pathogenesis of the disease is that obstruction leads to pain and malnutrition leads to pain. He puts forward the treatment principle of “promoting Qi and blood circulation， regulating deficiency and excess， and dredging the meridians”. While selecting acupoints， combining modern neuroanatomy with acupuncture and moxibustion theory， selecting acupoints accurately; and emphasizing acupuncture techniques and directions， and the spirit is adjusted to remove pain. Professor YANG also pays attention to the combination of part and whole， selects Taichong， Hegu， Zhongwan and other acupoints. In terms of treatment methods， a variety of comprehensive therapies are adopted. Electric acupuncture at Shangguan and Xiaguan points is used to strengthen acupuncture sensation; Xiaguan point is selected for warming acupuncture to enhance analgesic effect; wrist and ankle needles are indorsed after acupuncture to reduce the frequency of attack， with definite curative effect. In the medical case， the patient was diagnosed as Qi deficiency and blood stasis syndrome， both Qi and blood were supplemented to promote blood circulation and remove blood stasis， so that the head and face meridians were unobstructed and the patient‘s pain was removed automatically. [Conclusion] Professor YANG Jun‘s treatment of trigeminal neuralgia with acupuncture， combined with the pathogenesis of traditional Chinese and western medicines， reduces complex to simple，selects points on the interaction of Chinese and western medicine， pays attention to regulating spirit and treating as a whole. The treatment method pays attention to hybrid therapy， which greatly reduces the pain symptoms of patients， and the effect is quite good， which is worthy of clinical promotion. |
