| [目的] 探究俞震《古今医案按》按语中的脉学特点及思想，从而丰富中医脉诊的理论体系，为临床诊治疾病提供借鉴。 [方法] 以《古今医案按》中530余条按语及部分医案为研究对象，结合中医经典著作中相关论述，对俞氏按语中的脉学特色进行分析。 [结果] 分析认为，由于时代背景及师承缘故，俞氏辨证以脉诊为中心；根据脉象与药物的规律凭脉用药，同一患者脉象相同则病机未变，症同而脉不同考虑同病异治，若患者无证可辨时，脉诊为用药的可靠根据；判断预后方面，俞氏依据脉象转变评估病情顺逆；同时利用经络循行解释脉象变化，揭示脉象具有时间关联性。 [结论] 《古今医案按》按语中包含了丰富的俞氏脉学思想，为脉学理论提供了重要的指导和范例，值得借鉴学习。 |

| [Objective] To explore the characteristics and thoughts of pulse in YU Zhen‘s Notes of Record of Medical Cases in All Times， so as to enrich the theoretical system of pulse diagnosis in traditional Chinese medicine and provide reference for clinical treatment of diseases. [Methods] Taking more than 530 notes and some medical records in Notes of Record of Medical Cases in All Times， combined with the relevant expositions in the classic works of traditional Chinese medicine， the characteristics of pulse science in YU‘s notes were analyzed. [Results] The analysis shows that because of the background of the times and his teachers， the center of YU‘s syndrome differentiation is pulse diagnosis. YU took the pulse and medication according to the rules of the pulse and the drug， and revealed the same pulse of the same patient， the patients with the same symptoms and different pulses were considered the same diseases differently， if the patient had no syndrome for differentiation， the pulse diagnosis should be the reliable basis for medication. In terms of prognosis judgment， the condition was changed based on the pulse transition. At the same time， YU used meridians to explain the pulse changes， revealing that the pulse had time correlation. [Conclusion] There are abundant thoughts of YU‘s pulse in Notes of Record of Medical Cases in All Times， which provides important guidance and examples for pulse theory and is worth learning from. |
