| [目的] 增进对柴胡桂枝干姜汤病机和功效的认识，扩大其临床应用范围。[方法] 通过学习其他医家关于柴胡桂枝干姜汤的认识和论点，对柴胡桂枝干姜汤的原文论述、药物组成、配伍和功效进行分析和总结，结合诊疗风湿性疾病的临床实践，选用柴胡桂枝干姜汤治疗干燥综合征、结节性红斑的两则验案，详细叙述其诊疗过程并对医案加以按评，提出自己的认识和观点。[结果] 柴胡桂枝干姜汤具有清火润燥、转输津液、化湿散结的功效。针对干燥综合征郁火伤津、津失传输的特点和结节性红斑水结内停、水火互结的情况，分别运用柴胡桂枝干姜汤治疗，或清郁润燥，或散结利水，均取得较好的临床疗效。[结论] 临床上只要抓住柴胡桂枝干姜汤证火郁和水结两个关键病理特点，柴胡桂枝干姜汤可广泛应用于包括风湿性疾病等多种复杂病证的治疗。 |

| [Objective] To enhance the understanding of pathogenesis and function of Chaihu Guizhi Ganjiang Decoction， and expand the range of its clinical application. [Methods] By learning from other doctors‘ ideas and analyzing the original text， drug composition， drug compatibility and efficacy of Chaihu Guizhi Ganjiang Decoction， combining with clinical experience of rheumatic disease， two cases treated with this decoction were selected and evaluated in details， and main points of views were made. [Results] Chaihu Guizhi Ganjiang Decoction has the function of relieving internal heat， moistening dryness， transporting body fluid， resolving dampness and dispersing stasis， the pathogenesis characteristic of stagnated fire， insufficiency of liquid and dysfunction of liquid transporting of Sjogren‘s syndrome； and water retention inside and water fire interaction of erythema nodosum. In view of the two features above， by removing stagnated fire and moistening dryness， or dispersing retention and promoting diuresis， the curative effect has been achieved in rheumatic disease such as Sjogren‘s syndrome and erythema nodosum. [Conclusion] Focusing on the two pathological characteristics of fire stagnation and water retention， Chaihu Guizhi Ganjiang Decoction can be widely used in the treatment of various complex internal diseases including rheumatic disease. |
