[目的]观察推拿、中药外敷结合功能锻炼对膝骨性关节(knee osteoarthritison, KOA)患者股四头肌表面肌电信号(surface EMG signals, sEMG)及关节液P物质、白细胞介素-1β((interleukin, IL-1β)的影响,探讨其部分作用机制。[方法]将60例符合KOA纳入标准的患者按随机数字表法分为观察组和对照组,每组30例。KOA中医上一般属于寒湿痹阻型,对照予通痹散外敷结合股四头肌锻炼,观察组在此基础上予推拿手法治疗。两组疗程均为2周。比较两组患者股四头肌sEMG股外斜肌(vastus lateralis, VL)及股内斜肌(vastus medialis oblique, VMO)的积分肌电值(integral electromyography, IEMG)、中位频率(median frequency, MF)及关节液P物质、IL-1β含量。[结果]治疗后两组患者VL及VMO的IEMG、MF值较本组治疗前升高,关节液P物质、IL-1β含量较本组治疗前下降,差异均有统计学意义(P<0.05);组间治疗后比较,观察组对股四头肌sEMG,关节液P物质、IL-1β含量的改善优于对照组(P<0.05)。[结论]推拿、中药外敷结合功能锻炼能增强股四头肌的肌力及耐疲劳性,提高膝关节运动功能,降低关节液P物质、IL-1β含量可能是其作用机制之一。 |

[Objective] To observe effects of massage and external application of traditional Chinese medicine combined with functional exercise on surface EMG signals(sEMG), substance P and IL-1 Beta(IL-1β) of patients with knee osteoarthritison(KOA), and explore some mechanisms of it.[Method] 60 patients with KOA who met the inclusion criteria were randomly placed into control group and observation group, 30 patients in each group. The control group were accepted external application of Tongbi Powder and quadriceps exercises, while the observation group accepted massage on the basis of the control group. We compared the sEMG of quadriceps femoris[integral electromyography(IEMG) of vastus lateralis(VL) and vastus medialis oblique(VMO), median frequency(MF) and the contents of substance P and IL-1β in articular fluid between the two groups. [Result] After the therapy, the IEMG, MF value of VL and VMO were significantly higher than those of pre-treatment in the same one group(P<0.05), the contents of substance P and IL-1β in articular fluid were significantly lower than those of pre-treatment in the same one group(P<0.05); the observation group were improved better than those of control group on them(P<0.05).[Conclusion] Massage and external application of traditional Chinese medicine combined with functional exercise can enhance the muscle strength and fatigue resistance of quadriceps femoris, improve knee function, and its mechanism may be related to decrease of the content of substance P and IL-1β in joint fluid. |
