| [目的]分析总结“上火”与炎症之间的关系，探讨“上火”的生物学机制。[方法]在长期临床观察思考的基础上，通过计算机检索知网、万方、维普等数据库有关于“上火”和炎症的文献资料，进行综合分析，将“上火”与炎症反应状态下情志及内分泌的相关性进行挖掘，为“上火”的中西医研究奠定理论基础。[结果]传统医学认为的上火所表现的目赤肿痛、口干渴、口舌生疮、咽喉疼痛、小便短赤、大便秘结等各种症状与现代医学炎症急性反应期所呈现的局部红肿热痛以及功能失调相类似。从机理上看，“上火”与情志及内分泌密切相关，“上火”时体内多种炎症因子如IL-1、IL-6、TNF-α、CRP、PGE2等水平显著升高。[结论]炎症反应参与了中医上火的临床表现，是其发病的生物学机理之一。结合中医临床上火病症与现代医学炎症反应表现，通过检测多种炎症因子水平的相应变化，在一定程度上某些中医上火病症可能能够实现检测指标的标准化和客观化。 |

| [Objective]To analyze the relationship between internal heat and inflammation and discuss the biological mechanism of internal heat.[Methods]Through computer retrieval, the database of Zhiwang, Wanfang and Weipu has literature information about internal heat and inflammation, to conduct comprehensive analysis and explore the modern biological mechanism of internal heat.[Results] In traditional medicine, symptoms such as eye red swelling pain and tongue ulcer are similar to local red swelling hot pain and dysfunction in the acute reaction period of modern medical inflammation. From the perspective of mechanism, internal heat is closely related to emotion and endocrine, and is manifested as increased levels of inflammatory factors such as IL-1, IL-6, TNF-α, CRP and PGE2.That is, inflammation is involved in what is known in Chinese medicine as an inflammatory response. Among anxious and depressed people, tumor necrosis factor, c-reaction protein and some other inflammatory factors were obviously higher than normal. In addition, hypersensitivity of adrenal medulla, adrenal cortical function and increased release of inflammatory mediators were observed in people with cirrhosis. Additionally, PGE2 and PGF2α, TNF increased significantly in the crowd of people who got internal heat.[Conclusion] The inflammatory response is partly involved in the clinical manifestations of traditional Chinese medicine internal heat, and is one of the biological mechanisms of its pathogenesis. Further in-depth systematic study, there is a close correlation between internal heat-emotion-inflammation and internal heat-endocrine-inflammation. In combination with the clinical symptoms of traditional Chinese medicine internal heat and modern medical inflammation, by detecting the corresponding changes in the levels of various inflammatory factors, to a certain extent, some traditional Chinese medical internal heat disease may be able to achieve the standardization and objectification of detection indicators. |
