| [目的]探讨顾锡镇教授治疗颈动脉粥样硬化的临床经验。[方法]通过跟随顾锡镇教授抄方学习，收集相关病案，分析顾锡镇教授对颈动脉粥样硬化中医病因病机的认识，总结颈动脉粥样硬化的辨治经验，并附医案佐证。[结果]顾锡镇教授认为:肝肾阴虚、痰瘀阻络是颈动脉粥样硬化的主要病机，治法应以益肾养肝、化痰通瘀法为主。治疗上应注意平补肝肾、顾护正气，并结合现代中药药理学成果辨病用药，取效显著。所举验案中患者辨证为肝肾阴虚、痰瘀阻络证，治以益肾养肝、化痰通瘀，用药上重用制首乌，并根据复诊症状随证加减，诸症好转。[结论]顾锡镇教授采用益肾养肝、化痰通瘀法治疗颈动脉粥样硬化，疗效显著，值得进一步研究与推广。 |

| [Objective]To discuss the clinical experience of treating carotid atherosclerosis by Professor GU Xizhen. [Methods] Through consulting, studying and collecting the medical records, Professor GU Xizhen's academic point of view and clinical experience in treating carotid atherosclerosis from etiology and pathogenesis of disease were summarized and analyzed, and a related case was proposed. [Results] Professor GU Xizhen thinks that the pathogenesis of carotid atherosclerosis is “liver-kidney Yin deficiency and stagnation of phlegm and blood stasis”. The therapy for carotid atherosclerosis is “nourishing both liver and kidney, resolving phlegm and removing blood stasis”. Chinese medicine should be used calmly when nourishing liver-kidney Yin, and healthy-Qi should also be protected when resolving phlegm and removing blood stasis. Through referring to present-day pharmacological research, Chinese medicine based on disease differentiation should be paid attention to treat carotid atherosclerosis, thus the clinical curative effect was satisfactory. The patient in the case was diagnosed as “liver-kidney Yin deficiency and stagnation of phlegm and blood stasis”, treated by nourishing both liver and kidney, resolving phlegm and removing blood stasis. Professor GU Xizhen uses Radix polygoni multiflori preparata as the monarch herb, and changes the prescription according to the symptoms of the follow-up examination so that the symptoms were improved. [Conclusion]Professor GU Xizhen’s experiences were proved to be effective in treating carotid atherosclerosis with nourishing the liver and kidney, resolving phlegm and removing blood stasis. It deserves further study and promotion in clinical practice. |
