| [目的]总结范永升教授在临床上应用黄芪桂枝五物汤治疗硬皮病的经验，从而更好地指导临床实践。[方法]通过跟师和收集整理范老师应用黄芪桂枝五物汤治疗硬皮病的医案，分析范老师对硬皮病病因病机的认识以及对黄芪桂枝五物汤的方药分析，并列举范老师治疗硬皮病的一则经典案例。[结果]范老师认为硬皮病的病机为阳虚寒凝，肺脾不足，络脉痹阻，终致皮肤失养。选用温阳散寒，益气和血通痹之黄芪桂枝五物汤加减，适时佐以理气、化痰、疏肝解郁之法，切中病机，取得较好的临床效果。所举验案中患者辨证主要为寒凝血瘀之症，经过黄芪桂枝五物汤散寒活血通痹治疗之后，雷诺症状好转，诸证稳定。[结论]范永升教授从硬皮病病机出发，拟定温阳散寒、通络袪瘀、培补肺脾的治法方药，治疗卓有成效，值得深入学习和借鉴。 |

| [Objective] To summarize the experience of Professor FAN Yongsheng who uses the herbal prescription Huangqi Guizhi Wuwu Tang to treat scleroderma, so as to direct our clinical practice better. [Method] By collating Pro. FAN's medical cases that applying Huangqi Guizhi Wuwu Tang to the treatment of scleroderma, clearing and defining how teacher FAN thinks etiologies and pathogenesis of scleroderma and finding out the prescription of Huangqi Guizhi Wuwu Tang and giving one typical clinical case.[Result] Teacher FAN considers the basic pathogenesis is skin losing nourishing caused by Yang deficiency and coagulated cold and deficiency of spleen and lung. Therefore, Huangqi Guizhi Wuwu Tang of Wenyang dispersing cold, invigorating Qi and the circulation of Tongbi is often selected, together with the method of regulating Qi, resolving phlegm, soothing liver and relieving depression, aiming at the pathogenesis, and achieving good clinical effect. The syndrome differentiation of the patient in the case was mainly of cold coagulation and blood stasis. After the treatment of Huangqi Guizhi Wuwu Tang to dissipate cold and activate blood to Tongbi, the symptoms of Reno relieved and all the syndromes were stable. [Conclusion] Starting from the pathogenesis of scleroderma, Professor FAN Yongsheng formulates the treatment of warming Yang for dispersing cold, dredging collaterals for removing blood stasis, strengthening the lung and spleen, which is effective and worthy of further study and reference. |
