| [目的]总结舒琦瑾教授临证治疗肺癌的临床经验和学术思想。[方法]通过跟随舒琦瑾教授抄方侍诊，总结舒师运用中医治疗肺癌的临床诊疗思路及用药经验，并列举一则典型医案加以分析。[结果]舒师认为，肺癌由于伏气、六郁等引致发病，导致肺阴亏虚、内热蕴结，肺阴久虚，而致全身气虚；延及于脾，脾气亦亏虚。治法以养阴润肺为基础，以沙参麦冬汤滋阴养液、制肺虚火，合以四君子汤健脾益气；同时宜标本兼顾，予以清热解毒之药对结合南方红豆杉消痞抗癌、解毒祛邪。辨病与辨证结合，临床上治疗肺癌收效明显。所选医案中肺癌患者辨证属肺脾两虚、湿毒蕴阻，治以养阴益肺、清热解毒，以沙参麦冬汤合四君子汤加减治疗后，获得满意疗效。[结论]舒师治疗肺癌遵循益肺养阴、健脾益气的治疗原则，同时标本兼顾，辅以清热解毒之品，疗效确切，其治疗经验有较高临床参考价值，值得深入学习和推广运用。 |

| [Objective]To summarize the experience and academic thoughts of Professor SHU Qijin in treatment of lung cancer. [Methods]Through copying prescriptions and following Professor SHU Qijin’s clinical study, the main points of Professor SHU’s experience in herbs use and academic thoughts were summarized. Finally, a typical case was adopted as proof. [Results]Professor SHU indicates that lung cancer is aroused by hidden Qi, six depressions, which leads to lung Yin deficiency, internal heat accumulation, and deficiency of lung Yin lasts for a long time, affecting the generation of Qi, causing Qi deficiency in the body, extending to the spleen, and Qi of spleen is also deficiencies. Treatment bases on nourishing Yin and lung, and Shashen Maidong Decoction is used to nourish Yin and liquid, restrain the virtual fire of the lungs, combined with Sijunzi Decoction to replenish Qi and invigorate the spleen. At the same time, the symptoms are taken care of, and the couplet drugs for clearing away heat and removing toxicity are used combined with the Taxus wallichiana to eliminate cancer with combination of disease differentiation and syndrome differentiation, the clinically effect was obvious. In the selected medical case, the patient was differentiated as deficiency of both lung and spleen, dampness and toxin retention, and achieved satisfactory results after treated with addition and subtraction of Shashen Maidong Decoction and Sijunzi Decoction, according to nourishing Yin to moisten lung, replenishing Qi to invigorate the spleen, and clearing away heat and removing toxicity. [Conclusion] Professor SHU treats lung cancer according to the principles of nourishing Yin to moisten lung, replenishing Qi to invigorate the spleen, while taking care of the specimen, supplemented by clearing away heat and removing toxicity. The experience has high clinical reference value, which is worthy of in-depth study and application. |
