| [目的]总结温成平教授对系统性红斑狼疮(systemic lupus erythematosus，SLE)皮肤损害的辨治经验。[方法]通过系统阅读相关文献，临床跟师学习，从病名探讨、病机分析、辨证论治等方面总结温教授治疗SLE皮肤损害的临床经验及方药特色，并附验案一则。[结果]温教授认为，“毒、瘀、虚”为SLE皮肤损害的病机关键，治疗上强调以“解毒祛瘀滋阴”为主，临证综合运用凉血化斑、滋补肝肾、温经通脉等法，注重分期论治、标本兼顾，急性活动期宜清热解毒、凉血化斑；缓解期则以滋阴清热、益气养阴及祛瘀生新为主；结合中医外治法及日常调护，提高患者生活质量。所举验案辨证为阴虚内热、毒蕴血络，治以滋阴解毒、凉血消斑，予青蒿鳖甲汤加减，取得较好的效果。[结论]温教授运用解毒祛瘀滋阴法治疗SLE皮肤损害临床疗效显著，其经验值得临床推广和深入学习。 |

| [Objective] To summarize Professor WEN Chengping’s scholar thoughts and experiences of both syndrome differentiation and treatment of systemic lupus erythematosus(SLE) skin lesion.[Methods]Through clinical learning with the teacher and systematic reading literatures about SLE skin lesion, Professor WEN’s academic views and clinical experiences on treatment of the disease was summarized from the aspect of discussion of disease name, pathogenesis, syndrome differentiation and treatment. At the same time, one proven case was presented accordingly.[Results] Professor WEN believes that“poison, blood stasis, deficiency”is the key pathogenesis of SLE skin lesion; and emphasizes that the treatment is mainly to remove the toxicity, dissolve the stagnant and nourish Yin. The methods of cooling blood and removing spots, nourishing liver and kidney, warming meridians and dredging channels should be applied synthetically in clinical practice. Meanwhile it is necessary to pay attention to the staging treatment, give consideration to root and tip.In acute active stage, it is advisable to clear away heat, remove toxins and cool blood to remove spots; in remission stage, nourishing Yin, nourishing Qi and removing blood stasis and promoting tissue regeneration should be the main methods. In combination with the application of external treatment of TCM and daily care, the quality of life of patients can be improved. The patient was differentiated with SLE of Yin deficiency and heat toxin involving blood vessels, and achieved good results after treated with Qinghao Biejia decoction followed the treatment principle of nourishing Yin and detoxicating, cooling blood and eliminating spots. [Conclusion]Professor WEN has remarkable clinical efficacy in treating SLE skin lesion by detoxificating, removing blood stasis and nourishing Yin, which is worthy of application and in-depth study. |
