| [目的]介绍精神或情志异常类病证的中医经方诊疗思维。[方法]选取4例精神异常类病证经方治疗验案，根据患者的整体症状体征与精神、情志异常症状进行综合辨证，总结选取《伤寒杂病论》经方治疗精神或情志异常类病证的经验。[结果]病案一根据患者躁狂、涎液多、干呕等症，选用吴茱萸汤温肝散寒、温胃降逆，治疗抗精神病药物不良反应涎多一症，另辟蹊径，收效颇佳；病案二根据患者幻视幻听、脾气暴躁等症，辨证选用“病如狂状妄行，独语不休”的防己地黄汤加减，治疗老年痴呆躁狂症取效；病案三根据患者“昼日烦躁，夜而安静”的主症，结合其阴盛阳虚的症状，以干姜附子汤治疗双相抑郁，收效良好；病案四根据患者身疼重、烦躁等症，选用桂枝加黄芪汤治疗产后风湿阻遏阳气所致的烦躁症。[结论]辨证应用张仲景《伤寒杂病论》经方可以治疗精神或情志异常类病证，对临床精神类疾病的诊疗具有重要的启示。 |

| [Objective] To introduce the thinking of diagnosis and treatment of mental or emotional disorders with TCM classical prescriptions. [Methods] Select the typical cases of mental or emotional disorders, analyze the overall symptoms and signs of patients combined with syndrome of mental and emotional disorders, and select related prescriptions from the "Treatise on Febrile and Miscellaneous Diseases" for treatment. [Results] According to the symptoms of mania, saliva excess and retching, Wuzhuyu decoction was used in case 1 to warm the liver and disperse cold, warm the stomach and reduce adverse effects of antipsychotic drugs, which achieved a good effect. Based on symptoms of visual hallucination, auditory hallucination and irritability, Fangji Dihuang decoction was used to treat mania of senile dementia in case 2. The case 3 was diagnosed as bipolar depression with the symptoms of Yin flourish and Yang decline, combined with clinical feature of "restlessness day by day, quiet night by night", and Ganjiang Fuzi decoction was effective. Case 4 had severe pain and irritability, Guizhi Jia Huangqi decoction was selected to treat postpartum depression and dysphoria caused by Yang Qi arrest. [Conclusion] The classical prescription of ZHANG Zhongjing's“Treatise on Febrile and Miscellaneous Diseases” can treat mental and emotional disorders, which has important implications for the diagnosis and treatment of clinical psychiatric diseases. |
