| [目的]对比研究杠杆定位手法与斜扳法治疗腰椎间盘突出症(lumber disc herniation，LDH)的临床疗效与其对骨盆参数的影响。[方法]将符合纳入标准的67例LDH患者随机分为杠杆定位手法组(观察组)和斜扳法组(对照组)，分别将两组治疗前后的临床疗效、症状体征评分，骨盆左右倾斜度、左右髂嵴距离差、髂后上棘距离差进行评估比较。[结果]①临床疗效:治疗组总有效率优于对照组，差异有统计学意义(P<0.01)。②临床症状体征评分:两组评分较治疗前均改善，观察组改善优于对照组，差异均有统计学意义(P<0.05)。③骨盆参数:与治疗前比较，治疗后两组患者骨盆参数均转佳，差异有统计学意义(P<0.01)。治疗后两组间比较，观察组骨盆参数变化优于对照组，差异有统计学意义(P<0.05)，结果提示杠杆定位手法对骨盆参数的调节更佳。[结论]与斜扳法相比，杠杆定位手法能更有效地治疗腰椎间盘突出症，明显改善患者的临床症状与体征，且对腰椎间盘突出症患者的骨盆参数有较好的调节作用。 |

| [Objective] To study and compare the clinical efficacy and influence on pelvic parameters of lever positioning manipulation and oblique-pulling manipulation in treating lumbar disc herniation(LDH) .[Methods] 67 patients with LDH who met the criteria for inclusion were randomly divided into the lever positioning manipulation group(the observation group) and the oblique-pulling manipulation group(the control group). The clinical efficacy, the scores of symptoms and signs, the pelvic inclination, the distance of bilateral iliac crest，the distance of bilateral posterior superior iliac spine of two groups were evaluated and compared. [Results] ①Clinical efficacy: The total effective rate of the observation group was significantly better than that of the control group. The data of two groups were statistically different(P<0.01); ②The clinical symptoms and signs score:The score of patients in two groups improved after treatment(P<0.01). The score of the observation group increased more than that of the control group. And the scores of two groups were statistically different(P<0.05).③Pelvic parameters:Pelvic parameters of two groups improved compared with pre-treatment. The data of two groups were statistically different,P<0.01.After treatment，the efficacy of observation group was better than the control group,P<0.05.The results suggested that the lever positioning manipulation worked better in treating patients with abnormal pelvic parameters. [Conclusion]Compared with the oblique-pulling manipulation, lever positioning manipulation is very effective in treating LDH. It significantly relieves the symptoms and signs of patients，abnormal pelvic parameters and increases the stability of the lumbar structure. |
