| [目的]总结辨精在男性不育辨证中的应用。[方法]在复习历代医籍基础上，厘清各种“精”的确切含义，及其与男性生殖的关系。结合临床所见，寻找辨精的方法，并确定其辨证意义。[结果] “生殖之精”是男性得以正常生育的核心。辨“精”施治是男性不育辨治的关键所在。辨生殖之精是辨证论治男性不育的基础，常见的生殖之精病理表现包括生殖之精不足、不纯和不通。精液是生殖之精的重要外象，但二者并不等同，辨精的内容包括辨精液和辨精气，其内涵比辨精液更加广泛。[结论]由于缺乏足够的特异性，精液分析等实验室指标指导辨证的作用较差，传统四诊仍然是准确辨析“生殖之精”的有效方法。对于确属无证可辨者，辨病论治、投石问路是可行的有效方法。 |

| [Objective] To summarize the application of distinguishing the essence differentiation in the infertility of males. [Methods] Analyzing the meaning and the content of distinguishing the essence differentiation, searching out the way and the clinical meaning of distinguishing the essence differentiation according to the cognition of basic conception. [Result] The core of male progeny multiplication is “essence". So how to treat based on the essence differentiation is the key of syndrome differentiation on male infertility. Identifying the essence of reproduction is the basis of syndrome differentiation on male infertility. The deficiency, the impurity and the obstruction of the essence of reproduction are the common pathologic features. Semen is the important external representation of the essence of reproduction, but they are not equivalent. The content of distinguishing the essence is broader than the analysis of semen. [Conclusion]As a result of the lack of sufficient specificity, laboratory indexes such as semen analysis have poor effect in guiding syndrome differentiation. The traditional four methods of diagnosis continue to be the effective way to differentiate and analyze specifically. As for the patients who have no pattern to discriminate, treating according to disease differentiation and giving the prescription according to the characteristic of infertility and figuring syndrome out according to the clinical manifestations which show up afterward are the feasible and effective methods. |
