| [目的]总结马大正教授治疗妊娠恶阻的学术观点和临床经验。[方法]通过整理马大正教授临床治疗妊娠恶阻的医案及其出版和发表的有关妊娠恶阻的书籍和文章，介绍其诊治妊娠恶阻的临床经验，并举医案以例证。[结果]马老临床诊治妊娠恶阻者众多，纵观其医案，发现其中脾胃虚寒证、寒热错杂证、胃府郁热证、寒饮中阻及肝胃不和证为多见，其认为治疗关键在于辨证准确，治法合理，精确选方用药，才能取得良效。临证时，马老师通过辨证论治，方从法出，法随证立，治愈了无数妊娠恶阻患者。其认为脾胃虚寒者应治以温中补虚、调理脾胃；寒热错杂者应清温并用、和解寒热；胃府郁热者治以通腑泄热、和胃降逆；寒饮中阻者则治以温阳蠲饮、行气降逆；肝胃不和者应清肝疏气、和胃降逆。所举医案，具体展示了上述辨证思路和治疗经验。[结论]妊娠恶阻病因病机复杂，证型繁多，其中以脾胃虚寒证、寒热错杂证、胃府郁热证、寒饮中阻证及肝胃不和证多见，临证时马师通过详审病机，辨证论治，方随法变，常应手取效，其经验值得临床借鉴及推广。 |

| [Objective]To summarize the academic point of view and clinical experience of Professor MA Dazheng in diagnosing and treating hyperemesis gravidarum.[Method]Through sorting out the medical cases of Professor MA Dazheng's clinical treatment of hyperemesis gravidarum and the books and articles published on hyperemesis gravidarum,this paper introduces the clinical experience in the diagnosis and treatment of hyperemesis gravidarum and presents medical cases for verification.[Results]Teacher Ma has diagnosed and treated many patients with hyperemesis gravidarum in clinic，look at his medical records，we can find that spleen and stomach deficiency and cold syndrome，syndrome of combined with cold and heat，stomach-heat syndrome，the syndrome of cold drink in obstruction，disharmony between liver and stomach syndrome are more common，he believes that the key to treatment lies in accurate syndrome differentiation, reasonable treatment, accurate prescription medication, to achieve good results.In clinical practice, Teacher MA makes the treatment based on syndrome differentiation, the prescription comes from the law and the law is established with the syndrome, and has cured countless patients with hyperemesis gravidarum.In his view，spleen and stomach deficiency and cold syndrome should be treated by warming middle-Jiao and tonifying deficiency, regulating spleen and stomach; syndrome of combined with cold and heat should be treated by using clearing and warming simultaneously, coordinating cold and heat; stomach-heat syndrome should be treated by purging Fu-organs to eliminate heat，harmonizing stomach and lowering adverse Qi; the syndrome of cold drink in obstruction should be treated by warming Yang and promoting diuresis, regulating Qi and descending adverse flow of Qi; disharmony between liver and stomach syndrome should be treated by clearing liver heat and regulating Qi, harmonizing stomach and lowering adverse Qi.[Conclusion]The etiology and pathogenesis of hyperemesis gravidarum are complex, and its syndrome type is various, among them, spleen and stomach deficiency and cold syndrome, syndrome of combined with cold and heat, stomach-heat syndrome, the syndrome of cold drink in obstruction, disharmony between liver and stomach syndrome are more common, when in the clinic, Teacher MA through detailed examination of the pathogenesis, treating based on syndrome differentiation, and changing the prescription with the treatment, achieves good effects, so his experience is worthy of clinical reference and promotion. |
