| [目的]探讨夏永良主任治疗肝郁热毒型痤疮的临床经验。[方法]通过对夏永良主任门诊诊治肝郁热毒型痤疮的病例进行收集整理，分析夏主任治疗肝郁热毒型痤疮的诊治思路，并总结相关治疗经验及用药特色，最后通过分析典型案例进一步论证。[结果]夏永良主任认为肝郁热毒型痤疮的主要病机为肝气郁滞，肝郁化火，热毒壅滞，火毒相搏，攻于肌表，故而为疮。根据该病肝郁热毒的特点，主张以疏肝解郁、泻火解毒、凉血活血为治法。夏主任在治疗该类型痤疮时，立足辨证施治，以丹栀逍遥散合升降散治疗，并结合自己丰富的临证经验，灵活加减，取得了较好的临床疗效。所举验案为肝郁热毒型痤疮，治以疏肝解郁、泻火解毒、凉血活血，方用丹栀逍遥散合升降散加减，标本同治，疗效确切。[结论]夏永良主任认为治疗肝郁热毒型痤疮必须从整体出发，辨证辨病相结合，从“气、火、毒、瘀”多角度论治，其临床经验值得学习和推广。 |

| [Objective] To explore the clinical experience of Director XIA Yongliang in the treatment of acne of liver depression and heat toxin syndrome. [Methods] Through collecting the cases of acne of liver depression and heat toxin syndrome in the outpatient department of Director XIA, the thoughts of diagnosis and treatment of Director XIA were analyzed, and the relevant treatment experience and medication characteristics was summarized, and further demonstration was conducted through analysis of typical case.[Results]Director XIA Yongliang believes that the main pathogenesis of acne of liver depression and heat toxin syndrome is stagnation of liver Qi, fire derived from stagnation of liver Qi, obstruction of heat toxin, struggle of fire and toxin, finally leads to acne. According to the characteristics of the this disease, Director XIA advocates the treatment methods of dispersing stagnated liver Qi, purging fire and detoxification, cooling blood and promoting blood circulation. Based on syndrome differentiation, Director XIA treats this type of acne with Danzhi Xiaoyao San and Shengjiang San, modifies the prescriptions flexibly according to his rich clinical experience, and achieves great clinical results. The case was diagnosed as acne of liver depression and heat toxin syndrome, and got good results after treated with Danzhi Xiaoyao San and Shengjiang San to disperse stagnated liver Qi, purge fire and detoxification, cool blood and promote blood circulation.[Conclusion]Director XIA thinks that in the treatment of acne of liver depression and heat toxin syndrome, it is necessary to proceed from the whole, combine syndrome differentiation with disease differentiation, and treat from the perspectives of "Qi, fire, toxin, and stasis". Director XIA’s clinical experience is worth learning and promotion. |
