[目的]总结张萍青教授治疗子宫腺肌病的用药规律,并为中医治疗子宫腺肌病提供参考。[方法]纳入2011年1月至2016年1月张萍青教授门诊的113例子宫腺肌病病例,共640份医案,将患者临床信息录入临床方剂基础数据库,对药物频数、药性药味、归经采用描述性统计分析,对频数≥100的36味高频药物进行系统聚类及关联规则分析。[结果] 113例患者640份医案共用中药196种,累计用药频数11 630次,其中频数最多者为丹参,其后依次为忍冬藤、赤芍、仙茅、苁蓉、仙灵脾、蒲公英、阳春砂、鳖甲、熟地。药性使用频数最多的为“温”,药味使用频数最多的为“甘”。聚类分析将药物聚为5类,提炼出5对药对或药组:丹参与赤芍;地鳖虫与水蛭;皂角刺与鳖甲;仙茅与仙灵脾、苁蓉;忍冬藤与蒲公英、蛇舌草。[结论]张萍青教授认为子宫腺肌病的病因病机以肾虚为本,瘀毒为标,本虚标实,治疗上以扶正为本,温补先天为主,清化热毒、祛瘀止痛,且用药集中,善用药对。 |

[Objective] To summarize Professor ZHANG Pingqing’s medication rules and offer effective selections of Chinese medicine for clinicians in the treatment of adenomyosis.[Methods]Through collecting data from 640 cases of 113 adenomyosis patients from Chinese medicine clinic system during January 2011 to January 2016, clinical prescription underlying database was established. Descriptive analysis was applied to describe herb cumulative frequency, herb classification, nature and flavor, then cluster analysis and association rule analysis were adopted to analyze the 36 herbs of highest frequency. [Results]Totally 196 kinds of herbs were prescribed in 640 clinical medical cases of 113 patients. The cumulative frequency of herbs was 11 630, in which Radix Salvia Miltiorrhiza had the highest frequency, followed by Caulis Lonicerae Japonicae, Radix Paeoniae Rubra, Rhizoma Curculiginis, Cistanche deserticola, Epimedii Folium, Taraxacum mongolicum, Fructus Amomi Villosi, Carapax Testudinis, Rehmannia glutinosa. The highest frequency of nature and flavor is warm and sweet. The herbs were clustered into 5 groups through cluster analysis. Five medicinal pairs or medicinal groups were extracted: Radix Salvia and Radix Paeoniae Rubra, Polyphaga Sp. and Whitmania pigra Whitman, spiny spines and turtle carapace, Curculigo Orchioides Gaertn and Epimedium sagittatum, Cistanche deserticola Ma, Caulis Lonicerae Japonicae and Taraxacum mongolicum, Hedyotis Diffusa. [Conclusion] Professor ZHANG considers that the basic etiology and pathogenesis of adenomyosis is kidney deficiency and stasis toxin. Therapeutically, the treatment should be based on strengthening the body resistance, warming and tonifying the congenital, clearing heat and removing toxicity, removing blood stasis and relieving pain, and concentrated on medication, take good advantages of herb pairs. |
