| [目的]总结常诚教授应用通瘀泄毒益肾法治疗痴呆的经验。[方法]分析常师对痴呆内涵以及病机的认识，阐述通瘀泄毒益肾法在临床实践中的运用依据，分析经验方“通脑益智方”的组成、方义及功用，并附临床案例加以验证。[结果]常师认为痴呆病因病机虚实夹杂，虚者多由先天不足、肾虚精少、髓海不足、元神失养而发，实者多因毒瘀互结，破坏形体，损伤脑络而发，故"瘀血、浊毒、肾虚"是痴呆的主要病机。针对其病因病机，采用"通瘀泄毒益肾"治疗之法，创"通脑益智方"治疗本病。所附临床案例采用“通脑益智方”加减，一诊患者热毒内盛，以清热解毒除烦为主，二诊患者热毒渐退，夜寐不安，原方加用养心安神之品，三诊患者症状明显改善，通瘀泄毒益肾剂守方续治。全方共奏活血化瘀、清热解毒、补益肾精之效，切中病机，治疗痴呆取得满意的临床疗效。[结论] 常师运用通瘀泄毒益肾法，予“通脑益智方”加减治疗痴呆，疗效确切，值得进一步推广应用。 |

| [Objective]To summarize Professor CHANG Cheng's experience in treating dementia with the method of removing blood stasis, toxin and benefiting kidney. [Methods]To analyze the understanding of the connotation and pathogenesis of dementia by Dr. CHANG, to elaborate the application basis of the method of removing blood stasis, relieving poison and benefiting kidney in clinical practice, and to analyze the composition, meaning and function of "Tongnao Yizhi recipe", this was verified by clinical cases. [Results]Dr. CHANG thought that the etiology and pathogenesis of dementia were mixed with deficiency and excess, most of them were caused by kidney deficiency, deficiency of essence, deficiency of marrow and vital energy, and most of them were caused by toxin and blood stasis, destruction of form and injury of brain collaterals, so "blood stasis, turbid toxin and kidney deficiency" were the main pathogenesis of dementia. In view of its etiology and pathogenesis, the method of "dredging blood stasis and releasing toxin and tonifying kidney" was used to treat the disease. The attached clinical cases used "Tongnao Yizhi recipe" plus or minus. At the first diagnosis of the patient, heat and toxin were stored, mainly clearing away heat and detoxification and removing annoyance. At the second diagnosis, heat and toxin gradually retreated, but disturbed when sleeping, the original prescription added the product of nourishing the heart and calming the mind. At the third diagnosis, the symptoms were obviously relieved, and the prescription of dredging blood stasis, releasing toxin and tonifying the kidney was continued. The whole prescription had the effect of promoting blood circulation and removing blood stasis, clearing heat and detoxifying, tonifying kidney essence, cut off the pathogenesis and treated dementia with satisfactory clinical effect.[Conclusion]Doctor CHANG used the method of removing blood stasis, removing toxin and benefiting kidney to add or subtract the prescription of "Tongnao Yizhi recipe" to treat dementia. The curative effect was definite and worthy of further promotion and application. |
