| [目的]探析陆为民教授治疗口疮病的临床经验。[方法]通过门诊随师学习，整理陆为民教授相关医案，从病因病机、辨证思路及处方用药等方面分析总结陆为民教授治疗口疮病的临证经验，并附验案四则加以佐证。[结果]陆为民教授认为，口疮病之病标在口，本在脾胃。脾胃为后天之本，气血生化之源，脾胃升降功能失常，邪气侵犯口腔，气血壅滞，不得宣泄，发为口疮。脾胃不能升清降浊，是为本病的根本病机，或兼肝胆郁热、脾肾不足等。治疗上以运脾通降为基础，辅以协同治疗心肝肾，共同发挥功效。所举验案分别为外感风热、胆胃郁热、脾胃气虚、阴虚湿热四个证型，外感风热者治以疏风清热、运脾通降，胆胃郁热者治以清泄胆热、运脾通降，脾胃气虚者治以健脾升阳、运脾通降，阴虚湿热者则治以健脾化湿、滋阴降火、运脾通降，均获得较好的临床疗效。[结论]陆为民教授从脾胃升降的角度治疗口疮病，颇具特色，具有重要的临床价值。 |

| [Objective]To explore Professor LU Weimin's clinical experience in treating aphtha. [Methods] Through following outpatient work of Professor LU, collecting the medical records, the clinical experience of Professor LU in treating aphtha was analyzed and summarized from the aspects of etiology, pathogenesis, syndrome differentiation and prescription, and four cases were presented as proof. [Results] Professor LU believes that the symptoms of aphtha are ascribed to the spleen and stomach. Spleen and stomach are the acquired foundation and the origin of Qi and blood. Ascension and descent function of spleen and stomach is abnormal, exogenous pathogens invade the mouth, Qi and blood stagnate, then leads to aphtha. Disfunction of ascending lucidity and descending turbidity of spleen and stomach was the fundamental pathogenesis of this disease, sometimes combined with the internal heat of liver and gallbladder, deficiency of spleen and kidney. The treatment is based on activating spleen and regulating descending, and synergistic therapy of heart, liver and kidney. Cases of external wind heat syndrome, stagnated heat of gallbladder and stomach syndrome, deficiency of Qi of spleen and stomach syndrome and Yin deficiency and damp-heat syndrome were presented. Patient with external wind heat was treated with dispersing wind and clearing heat, activating spleen and regulating descending; patient with stagnated heat of gallbladder and stomach was treated with clearing and purging gallbladder heat, activating spleen and regulating descending; patient with deficiency of Qi of spleen and stomach was treated with invigorating spleen and promoting Yang, activating spleen and regulating descending; patient with Yin deficiency and damp-heat was treated with invigorating spleen to dissipate dampness, nourishing Yin and reducing fire, activating spleen and regulating descending. All patients achieved good results.[Conclusion] Professor LU's methods of activating spleen and regulating descending has significant clinical value in treating common aphtha. |
