      [目的]探讨肾病Ⅰ号方对肾间质纤维化大鼠肾组织转化生长因子-β1(transforming growth factor-β1,TGF-β1)和结缔组织生长因子(connective tissue growth factor,CTGF)表达的影响,以及抗肾间质纤维化的可能机制。[方法]选取SPF级健康成年雄性SD大鼠72只,随机分为假手术组、模型组、贝那普利组和肾病Ⅰ号方组,每组18只。除假手术组外,其余大鼠建立单侧输尿管梗阻(unilateral ureteral obstruction,UUO)模型,术后第2天开始给予对应药物灌胃,贝那普利组给予贝那普利0.8mg/(kg·d)灌胃,肾病Ⅰ号方组则予肾病Ⅰ号方30g/(kg·d)灌胃,均为2次/d,共3周。检测术后第7、14、21天各组大鼠肾功能及肾组织病理学改变,应用免疫组化检测TGF-β1和CTGF在肾组织的表达情况。[结果]与假手术组比较,模型组和两个药物处理组第7、14、21天血肌酐、尿素氮水平均明显升高(P<0.05);与模型组比较,两个药物处理组血肌酐、尿素氮水平明显降低(P<0.05)。Masson染色提示,与假手术组比较,模型组第7、14、21天肾间质纤维化指数均明显升高,两组之间差异均有统计学意义(P<0.05),而贝那普利组和肾病Ⅰ号方组各时间点的肾间质纤维化指数均明显低于模型组(P<0.05),两个药物处理组之间差异无统计学意义(P>0.05)。与假手术组比较,第7、14、21天模型组和两个药物处理组TGF-β1、CTGF表达水平均明显升高(P<0.05);与模型组比较,贝那普利组或肾病Ⅰ号方组的表达水平均明显降低(P<0.05);两个药物处理组之间差异则无统计学意义(P>0.05)。[结论]肾病Ⅰ号方通过抑制肾小管上皮细胞TGF-β1、CTGF的表达,减少细胞外基质的沉积,从而起到保护肾小管、抗肾间质纤维化的作用。

      [Objective]To investigate the influence of Nephropathy 1st Decoction on the expression of transforming growth factor-β1(TGF-β1) and connective tissue growth factor(CTGF) in rats with renal interstitial fibrosis and the possible mechanism.[Methods] Seventy two healthy adult male SD rats of SPF grade were randomly divided into sham group, model group, Benazepril group, Nephropathy 1st Decoction group, with 18 in each group. Except the rats in sham group, rat model of renal interstitial fibrosis were established by unilateral ureteral obstruction(UUO), the corresponding drugs was administered on the 2nd day after operation, twice a day by gavage administration, Benazepril group was gavage administrated with 0.8mg/(kg·d)Benazepril, Nephropathy 1st Decoction group was gavage administrated with 30g/(kg·d)Nephropathy 1st Decoction, for 3 weeks. The renal function and renal pathological changes were observed on the 7th, 14th and 21st day after operation. The protein expression of TGF-β1 and CTGF in renal tissue was detected by immunohistochemistry.[Results]Compared with sham group, the level of serum creatinine(Scr) and blood urea nitrogen(BUN) increased significantly in the other three groups(P<0.05). Compared with model group, the level of Scr and BUN decreased in Benazepril group and Nephropathy 1st Decoction group(P<0.05).Masson staining showed that renal interstital fibrosis index of model group was higher than sham group(P<0.05), while the renal interstitial fibrosis index of Benazepril group or Nephropathy 1st Decoction group was significantly lower than that of model group(P<0.05). Compared with sham group, the expression of TGF-β1 and CTGF was significantly higher in model group and two treatment group(P<0.05).Compared with model group, the expression of TGF-β1 and CTGF in Benazepril group or Nephropathy 1st Decoction group were lower(all P<0.05), and there were no significant differences between Benazepril group or Nephropathy 1st Decoction group(P>0.05).[Conclusion]Nephropathy 1st Decoction plays a protective role in kidney,which reduces the accumulation of extracellular matrix through inhibiting the expression of TGF-β1 and CTGF in renal tubular epithelial cells may be the possible mechanism.
