[目的] 分析国医大师葛琳仪全程管理肺病的用药规律。 [方法] 纳入2013年9月1日至2018年8月31日期间葛琳仪门诊咳嗽、哮病、肺胀患者共117例604诊次,采集录入患者临床信息,建立临床方剂数据库,对药物频数、性味采用描述性分析,对频数最高的30种药物进行系统聚类分析以及药物关联分析。 [结果] 117例患者604诊次共用药212种,咳嗽、哮病、肺胀用药频数最高者分别为前胡、徐长卿、陈皮,三病用药中药性寒、温、平,药味苦、辛、甘均高频数使用。聚类分析涉及经典方剂三子养亲汤、二仙汤、生脉散、麻杏石甘汤等。药物关联分析得出桔梗、连翘、金银花、黄芩、蒲公英与咳嗽相关,徐长卿、葶苈子、黄芩、苦杏仁、蒲公英、紫苏子、金银花、连翘与哮病相关,陈皮、葶苈子与肺胀相关。[结论] 葛琳仪运用整体的全程管理思维,结合“治未病”理论,把握肺病发展各阶段病机特点,重视既病防变,治肺同时兼顾脾肾,以“宣肺”为要点,“清法”为总纲,用药轻灵,标本兼治。 |

[Objective] To analyze national medical master Professor GE Linyi’s experience in overall management and treatment of lung disease. [Methods] Record 117 patients with cough, asthma, lung distension visiting Ge’s clinic, 604 clinical visits in total, were collected. Clinical information was collected and clinical prescription was established to conduct descriptive analysis on the cumulative frequency, properties and flavors of medicinal. Also cluster analysis and drug association analysis were applied to 30 medicines with the highest frequency. [Results] Totally 212 medicines were prescribed in 604 clinical visits with 117 patients. The highest frequency drugs of cough, asthma and lung distension were Qianhu, Xuchangqing and Chenpi respectively. The high frequently used medicinal properties were cold, warm and flat drugs, and the flavors of medicines were bitter, pungent and sweet. The medicines were classified by cluster analysis, involving formulas including Sanzi Yangqin Decoction, Erxian Decoction, Shengmai San, Maxing Shigan Decoction, etc. Drug association analysis concluded that platycodon, forsythia, honeysuckle, scutellaria, dandelion were associated with cough; Xuchangqing, draba nemorosa, scutellaria, bitter almond, dandelion, Perilla, honeysuckle, forsythia were associated with asthma. Chenpi and draba vari were associated with lung distension. [Conclusion] GE Linyi used the overall management thinking, combined with the theory of "preventive treatment of disease", to grasp the pathogenesis characteristics of lung disease at all stages of its development, attached importance to preventing disease from change, treating lung while giving consideration to spleen and kidney, taking "promoting lung" as the main point and "clearing method" as the general outline, using drugs with light and flexible efficacy, and treating both symptoms and root causes. |
