| [目的]浅析仲景“固小便以通大便”论治便秘的学术思想，丰富中医药论治便秘相关疾病的理论基础。[方法]遵“以经解经”的基本原则，对比《伤寒论》《金匮要略》涉及“固小便以通大便”论治便秘的相关条文，结合近代文献研究及临床验案，解析仲景以“固小便以通大便”论治便秘的学术思想。[结果]仲景对二便之间关系的研究颇具心得，虽未明确提及“固小便以通大便”的学术观点，但应用该思想论治便秘的证据确切存在，且临床验证亦能取效。其主要包括以下三个方面:温阳祛湿、固津通便；泄热润肠和补肾温阳、固津通便。在该思想的指导下，以肾气丸合黄芪桂枝五物汤加减治疗某八旬中风后遗症期便秘患者，取效颇佳。[结论]深入理解、思考仲景“固小便以通大便”的学术思想，对临床治疗便秘有着重要的指导意义。 |

| [Objective]To analyze the adaption of ZHANG Zhongjing's academic thoughts of "arrest polyuria to treat constipation" in treating constipation, and enrich the theoretical basis of TCM in treating constipation-related diseases.[Methods]According to the basic principle of "interpreting classics by classics", through comparing the original texts of "Treatise on Febrile Diseases" and "Synopsis of the Golden Chamber" concerning the treatment of constipation by "arrest polyuria to treat constipation", with the modern literature research and clinical trials, Zhongjing's academic thought of treating constipation by "arrest polyuria to treat constipation" was analyzed.[Results]Through literature review, it was found that Zhongjing had a lot of experience in the study of the relationship between urine and stool, among which the academic thoughts of "arrest polyuria to treat constipation" in the treatment of constipation was more obscure. Although Zhongjing did not explicitly mention the academic viewpoint of "arrest polyuria to treat constipation", the evidence of application of this thoughts in treating constipation existed, and clinical verification was effective. Zhongjing's thoughts of "arrest polyuria to treat constipation" mainly includes the following aspects: Warming Yang and removing dampness, protecting body fluid to treat constipation; relieving heat and moistening intestines; tonifying kidney and warming Yang, protecting body fluid to treat constipation. Under the guidance of this thoughts, Shenqi Pill and Huangqi Guizhi Wuwu Decoction was proved to be effective in the treatment of constipation of eighty-year-old patient with the sequelae of apoplexy.[Conclusion]It has important guiding significance for clinical treatment of constipation to thoroughly understand and reflect Zhongjing's academic thoughts of "arrest polyuria to treat constipation". |
