| [目的]归纳总结徐力教授治疗卵巢癌的临床经验。[方法]通过临床跟师学习，收集并整理相关病例和资料，从卵巢癌的病因病机、分期辨证等方面，介绍徐力教授的临证经验及遣方用药思路。并列举验案一则，加以佐证。[结果]徐力教授认为，卵巢癌的主要病因病机为先天不足，或久病伤正，正气虚损，脏腑功能紊乱，冲任、气血失调，或情志内伤、饮食不节等所致从而导致寒凝、气滞、血瘀、痰湿、毒热蕴结，内生癌毒，积聚于胞宫而成。根据卵巢癌不同的治疗阶段可分为3期:辅助治疗期(包括围手术期、围化疗期、围放疗期的治疗)分别以气血亏虚、气阴两虚、阴虚生热为主，治当补益气血、健脾和胃、清热解毒；维持治疗期以正气不足为主，兼有癌毒内盛之候，治当扶正固本、滋阴填髓、抗癌解毒；姑息治疗期以脏腑盛衰、气血阴阳亏虚的症状为主，重在固本培元，顾护胃气，兼以解毒消癥。并结合“六辨”即辨病变部位、病理类型、指标水平、病机特点、证候虚实、兼夹症状，实施精准抗癌，达到辨病辨证相结合的目的。且以分龄论治作为补充。所举医案，患者晚期，恶液质状态，气血亏虚，水瘀互结，癌毒内盛，治当先以补益气血、健脾和胃，利湿行水为主，后兼顾解毒消癥之力，以“黄芪防己汤”加减治疗，疗效甚著。[结论]徐力教授运用“三段六辨”模式治疗卵巢癌的思路，见解独特，可取得良好效果，值得进一步研究与推广。 |

| [Objective] To summarize the clinical experience of Professor XU Li in the treatment of ovarian cancer.[Methods] The clinical experience of Professor XU Li and his thoughts on prescription and medication were introduced from the aspects of etiology, pathogenesis and prescription of ovarian cancer. A proved case was presented.[Results] Professor XU Li believes that according to different treatment stages of ovarian cancer, it can be divided into three stages: In auxiliary treatment period (including perioperative period, peri-chemotherapy period, and peri-therapeutic period), with Qi and blood deficiency, Qi and Yin deficiency, heat formation from Yin deficiency is the main reason for treating Qi and blood, strengthening the spleen and stomach, clearing away heat and detoxification; in maintaining the treatment period with the lack of vital Qi and stagnation of cancer, support the healthy energy and nourishing the Yin myeloid, anti-cancer detoxification; in palliative treatment period with visceral stagnation, Qi and Yin and Yang deficiency, focus on supporting Qi, care for stomach and detoxification and elimination. Combined with "six discrimination", it is to distinguish the lesions, pathological types, index levels, pathogenesis characteristics, syndromes, and symptoms, and implement accurate anti-cancer, and achieve a combination of disease differentiation and syndrome differentiation. And he uses the treatment by age as a supplement. The medical case, the patient's late stage, the dyscrasia state, with Qi and blood deficiency, the phlegm and blood stasis, the cancer toxin accumulated in the body, the treatment is first to replenish Qi and blood, strengthen the spleen and stomach, and then make detoxification and elimination. The disease is treated with the "Huangqi Fangji Decoction "plus or minus, and the curative effect is very good. [Conclusion] Professor XU Li using the "three periods and six differentiations" model to treat ovarian cancer shows the unique insights and can achieve good results, which deserves further research and promotion. |
