| [目的]介绍黄桂成教授从络病论治膝骨关节炎的辨证特点与用药经验。[方法]通过跟随黄桂成教授侍诊抄方，整理医案、临证揣摩，总结黄桂成教授从络病角度论治膝骨关节炎的经验，并附验案一则加以佐证。[结果]黄桂成教授认为筋骨由络脉充养，络脉不通是膝骨关节炎的病机本质，临证对于膝骨关节炎的辨证立足络病理论，从发病因素、病邪新旧、阴阳表里、寒热虚实、气病血病等方面着手，治疗上提倡“以通为用”，并善于运用辛味药、虫类药、藤类药、补益药等药通络蠲痹，临床疗效突出。所举医案患者辨为肝肾亏虚、寒湿阻络证，黄教授从“络”论治，以补益肝肾、温经通络为法，注重通络药与其他药物的灵活运用，经治后患者多年痼疾告愈。[结论]黄桂成教授诊疗膝骨关节炎，从“络”论治，强调辨证与用药的统一，思路清晰，特色突出，疗效明显，值得深入研究和推广。 |

| [Objective]To summarize Professor HUANG Guicheng’s experience in syndrome differentiation and medication of knee osteoarthritis from the perspective of collateral disease theory.[Methods]Through following Professor HUANG Guicheng’s clinical study, collecting and summing up the classical medical records of knee osteoarthritis, Professor HUANG’s experience in the treatment of knee osteoarthritis from the perspective of collateral disease were summarized, and one case was presented as proof.[Results] Professor HUANG Guicheng considers that the muscles and bones are filled and nourished by the collaterals, the essence of pathogenesis of knee osteoarthritis is blockage of collaterals. Based on collateral disease theory, the clinical syndrome differentiation of knee osteoarthritis should start from the aspects of pathogenic factors, new and old pathogenic factors, exterior and interior of Yin and Yang, deficiency and excess of cold and heat, Qi disease and blood disease, etc.In the treatment, Professor HUANG advocates “taking unblocked transmission as normal functional state”, and is good at using pungent drugs, insect drugs, rattan drugs, tonic drugs and other drugs to unblock collaterals and arthralgia, with outstanding clinical effects.The patient in the case was differentiated as deficiency of liver and kidney, cold and dampness blocking collaterals. According to collateral disease theory, Professor HUANG took tonifying the liver and kidney, warming the meridians and unblocking the collaterals as the method, attached great importance to the flexible use of collaterals drugs and other drugs, and after treatment, the patient recovered from the disease.[Conclusion] Professor HUANG Guicheng’s diagnosis and treatment of knee osteoarthritis is based on collateral disease theory, emphasizing the unity of syndrome differentiation and medication, combined with clear thinking, prominent characteristics and obvious curative effect, which is worth further study and promotion. |
