[目的]探讨中医药治疗系统性红斑狼疮(systemic lupus erythematosus,SLE)皮肤损害的用药规律及内在机制。[方法]通过整理中国知网中有关数据,筛选出治疗SLE皮肤损害的医案,采用统计软件SPSS进行数据统计,运用中医传承辅助平台(traditional Chinese medicine inheritance support platform,TCMISS)V2.5对SLE皮肤损害的用药特点进行分析,并运用中药系统药理学分析平台(traditional Chinese medicine systems pharmacology database and analysis platform,TCMSP)、人类基因数据库(GeneCards)、人类在线孟德尔遗传(Online Mendelian Inheritance in Man,OMIM)等数据库以及R语言、生物信息分析软件官方版(V3.6.1)等软件进行机制分析。[结果]共筛选从1979年1月1日至2019年3月14日的1 930篇文献,获取276首处方,基于关联规则分析得出生地黄-牡丹皮、生地黄-赤芍和牡丹皮-赤芍组合出现频次较高。基于复杂系统熵聚类的组方规律,生成9个SLE皮肤损害新处方,且新处方可能通过钙信号通路、环磷腺苷(cyclic adenosine monophosphate,cAMP)信号通路、胆碱能突触、缺氧诱导因子-1(hypoxia inducible factor-1,HIF-1)信号通路、白介素-17(interleukin-17,IL-17)信号通路、核因子-κB(nuclear factor-κB,NF-κB)信号通路、p53信号通路、磷脂酰肌醇-3-激酶-蛋白激酶B(phosphatidylinositol-3-kinase-protein kinase B/Akt,PI3K-PKB/Akt)信号通路、肿瘤坏死因子(tumor necrosis factor,TNF)信号通路等发挥作用。[结论]SLE皮肤损害整体治则以清热凉血解毒为主,常配合活血、补虚等治法。常用生地黄、牡丹皮、赤芍等药物,药物主要通过钙信号通路、cAMP信号通路等发挥作用。本研究通过总结SLE皮肤损害的中药用药规律,为深入分析验证SLE皮肤损害的中医药治疗提供了方向。 |

[Objective]To explore the medication regularity of traditional Chinese medicine in treating systemic lupus erythematosus(SLE) skin damage. [Methods]Through organizing the data about SLE in China National Knowledge Infrastructure(CNKI), the medical cases of SLE skin damage were screened out, and statistical software SPSS was used for data statistics. The characteristics of SLE skin damage were analyzed using the traditional Chinese medicine inheritance support platform(TCMISS,V2.5), and the mechanism was analyzed by traditional Chinese medicine systems pharmacology database and analysis platform(TCMSP), Human gene database(GeneCard), Online Mendelian Inheritance in Man(OMIM) database, and R programming language, Cytoscape software.[Results]From January 1, 1979 to March 14, 2019, totally 1 930 literatures were screened, and 276 prescriptions were obtained.According to the association rules analysis, the occurrence frequency of the combination of Radix Rehmanniae-Paeonia Moutan, Radix Rehmanniae-Radix Paeoniae Rubra, and Paeonia Moutan-Radix Paeoniae Rubra are relatively high.Based on the formula of complex system entropy clustering, nine SLE skin damage new formulas were generated. New formulas may act through calcium signaling pathway, cyclic adenosine monophosphate(cAMP) signaling pathway, cholinergic synapse, hypoxia inducible factor-1(HIF-1) signaling pathway, interleukin-17(IL-17) signaling pathway, nuclear factor-кB (NF-κB)signaling pathway, p53 signaling pathway, phosphatidylinositol-3-kinase-protein kinase B/Akt(PI3K-PKB/Akt) signaling pathway, tumor necrosis factor (TNF) signaling pathway and so on. [Conclusion]The treatment of SLE skin damage is mainly based on clearing away heat, cooling blood, and detoxifying, combined with promoting blood circulation and tonifying deficiency. Radix Rehmanniae, Paeonia Moutan and Radix Paeoniae Rubra were commonly used. Drugs mainly work through 9 related signaling pathways such as calcium signaling pathway and cAMP signaling pathway. This study provides a direction for in-depth analysis and verification of traditional Chinese medicine treatment of SLE skin damage by summarizing the medication regularity of traditional Chinese medicine treatment of SLE skin damage. |
