      [目的]探究不同饮食宣教对维持性血液透析终末期肾脏病患者内环境的影响,为临床合理选择饮食宣教方式提供依据。[方法]将65例血液透析患者随机分为传统饮食宣教组(对照组)32例与中医饮食宣教组(观察组)33例,分别给予传统饮食宣教与中医饮食宣教。分别于宣教前及宣教12个月后收集两组患者透析前血液样本,检测各样本的生化指标高密度脂蛋白(high density lipoprotein,HDL)、低密度脂蛋白(low density lipoprotein,LDL)、血尿素氮(blood urea nitrogen,BUN)、血肌酐(serum creatinine,SCr)、总胆固醇(total cholester,TC)、血钙、血磷、白蛋白和超敏C反应蛋白(hypersensitive C-reactive protein,hs-CRP);细胞因子肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、白介素-1β(interleukin-1β,IL-1β)、白介素-6(interleukin-6,IL-6)以及肠黏膜通透性指标晚期氧化蛋白产物(advanced oxidation protein products,AOPP), β2-微球蛋白(β2-microglobulin,β2-MG)、内毒素和分泌型免疫球蛋白A(secretory immunoglobulin A,sIgA)的水平。[结果] 与对照组比较,观察组患者宣教后LDL、SCr、血磷、AOPP、β2-MG、内毒素、hs-CRP水平下降,差异有统计学意义(均P<0.05);血钙、白蛋白、sIgA水平略有增加,差异有统计学意义(均P<0.05);HDL、BUN、TC、TNF-α、IL-1β和IL-6水平差异无统计学意义(P>0.05)。[结论]相比配合传统饮食宣教,血液透析治疗配合中医饮食宣教更能改善终末期肾脏病患者的内环境。

      [Objective]To investigate the effects of different diet education on the internal environment of end-stage renal disease(ESRD)patients with maintenance hemodialysis and furtherly provide the clinical basis for rational choice of diet education. [Methods]Sixty-five ESRD patients with maintenance hemodialysis were randomly divided into two groups: traditional diet education group(control group) , and Chinese medical diet education group(observation group), which were given traditional diet education and Chinese medical diet education, respectively. Blood samples of two groups before hemodialysis were collected before and 12 months after intervention, and the levels of biochemical indexes, including high density lipoprotein(HDL),low density lipoprotein(LDL), blood urea nitrogen(BUN),serum creatinine(SCr), total cholester(TC), serum calcium,serum phosphate, albumin and hypersensitive C-reactive protein(hs-CRP); cytokines including tumor necrosis factor-α(TNF-α), interleukin-1β(IL-1β) and interleukin-6(IL-6) and intestinal permeability index advanced oxidation protein products (AOPP), β2-microglobulin(β2-MG), endotoxin and secretory immunoglobulin A(sIgA) were detected.[Results] Compared with control group, the levels of LDL, SCr, serum phosphate, AOPP, β2-MG, endotoxin and hs-CRP decreased in observation group(all P<0.05), while the levels of serum calcium, albumin and sIgA increased(all P<0.05). There were no significant difference in the levels of HDL, BUN, TC, TNF-α, IL-1β and IL-6(P>0.05). [Conclusion]Compared with traditional diet education, Chinese medicine education diet can improve the internal environment of ESRD patients with maintenance hemodialysis.
