[目的]总结Nod样受体蛋白3(nod-like receptor pyrin domain 3,NLRP3)炎症小体在治疗痛风性关节炎(gouty arthritis,GA)中的作用。[方法]检索2015年1月至2020年1月Web of Science、PubMed、中国知网(China National Knowledge Infrastructure,CNKI)、维普、万方等数据库中中药通过NLRP3炎症小体干预GA的相关国内外文献,归纳总结出NLRP3炎症小体在中药治疗GA中的作用。[结果]共检索到相关中文文献35篇、英文文献45篇。分析文献发现,GA患者关节滑膜中NLRP3蛋白表达升高。中药治疗GA多以脾虚湿热瘀阻为辨证要点,通过影响NLPR3炎症小体,在GA治疗中发挥重要作用。NLRP3炎症小体由NLRP3支架、凋亡相关斑点样蛋白(apoptosis-associated speck-like protein containing a CARD,ASC)和效应蛋白半胱氨酸天冬氨酸特异蛋白-1(cysteinyl aspartate specific proteinase-1,caspase-1)三部分组成。以NLRP3炎症小体为核心,中药复方、单味中药提取物、中药单体均能够通过多个信号通路干预GA的发生和发展。[结论]以NLRP3炎症小体为切入点,深入探究中药治疗GA的具体机制,开发高效低毒的新药是未来研究的重要方向。 |

[Objective] To summarize the intervention effect of nod-like receptor pyrin domain 3(NLRP3) inflammasome in the treatment of gouty arthritis(GA). [Methods] From January 2015 to January 2020, the databases of Web of Science, PubMed, China National Knowledge Infrastructure(CNKI), VIP and Wanfang databases were searched for relevant domestic and foreign literatures on the intervention of NLRP3 inflammasome in GA by traditional Chinese medicine(TCM), and summarized the role of NLRP3 inflammasome in the treatment of GA by TCM. [Results] Totally 35 Chinese and 45 English literatures were retrieved. After analyzing the literatures, it is found that the expression of NLRP3 protein in synovium of GA patients is increased. TCM treatment of GA mostly takes spleen deficiency, dampness, heat and stasis as the main point of syndrome differentiation, and TCM plays an important role in the treatment of GA by affecting NLRP3 inflammasome. NLRP3 inflammasome is composed of NLRP3 scaffold, apoptosis-associated speck-like protein containing a CARD(ASC) and effector protein cysteinly aspartate specific proteinase-1(caspase-1). With NLRP3 inflammasome as the core, Chinese herbal compound, Chinese herbal extract and Chinese herbal monomer can intervene the occurrence and development of GA through multiple signaling pathways.[Conclusion] Taking NLRP3 inflammasome as the breakthrough point, it is an important direction for future research to explore the specific mechanism of TCM in the treatment of GA and develop new drugs with high efficiency and low toxicity. |
