| [目的] 介绍王莉莉主任医师运用动刺疗法为主的方案治疗顽固性肩周炎的临床经验。[方法] 通过跟师学习、临床教学、整理病例以及查阅文献，归纳总结王师治疗本病的学术观点和临证方案，并举医案佐证。[结果] 王师认为顽固性肩周炎由肩周炎粘连期未愈发展而来，肩部长期气血瘀滞，致脉络反复痹塞不通、关节不利。临证倡导以针灸动刺疗法、运动针法为主，推拿、功能锻炼为辅的治疗方案，该方案的关键在于精确定位痛点，增强针刺的针对性，能够大幅改善肩关节功能。本医案为双肩患病，辨为风寒痹阻证，首先施以运动针法畅通肩部经脉气机，再以动刺疗法精准定位痛点后动肩留针，配合艾灸、电针等手段，辅助推拿、功能锻炼，疗效持久显著。[结论] 王师运用动刺疗法为主的方案治疗顽固性肩周炎，不仅可在当下大幅改善肩关节功能，且远期疗效显著，不易复发，其经验值得借鉴并推广。 |

| [Objective] To introduce Professor WANG Lili‘s clinical experience in the treatment of refractory scapulohumeral periarthritis with dynamic needling therapy. [Methods] Through learning， clinical teaching， sorting out cases and reviewing literature， the academic viewpoints and clinical plan of Professor WANG were summarized， and a medical case was cited. [Results] Professor WANG believes that refractory scapulohumeral periarthritis is developed from the adhesion period of scapulohumeral periarthritis， which is caused by long-term severe Qi and blood stasis， resulting in repeated blockage of main and collateral channels and adverse joints. Clinically， the treatment scheme based on dynamic needling therapy， motion needling method， supplemented by massage， functional exercise is advocated. The key of the scheme is to locate the pain point accurately， enhance the pertinence of acupuncture， and then greatly improve the shoulder joint function. This case was a disease of both shoulders， which was identified as wind-cold obstruction syndrome. Firstly， motion needling method was applied to unblock Qi movement of the shoulder meridian， then the dynamic needling therapy was used to accurately locate the pain point， combined with moxibustion， electroacupuncture and other means， assisted by massage and functional exercise， and the curative effect was lasting and significant. [Conclusion] Professor WANG’s treatment of refractory scapulohumeral periarthritis by dynamic needling therapy can not only improve shoulder joint function significantly， but also has significant long-term effect and is not easy to relapse. Her experience is worthy of reference and promotion. |
