| [目的] 阐述和总结陆为民教授辨证治疗胆胃同病的相关学术思想、临床思路和经验。[方法] 通过跟师临诊，查阅古籍，总结病案资料，从病因病机，遣方用药等方面论述陆师治疗胆胃同病的经验，并佐以临床验案。[结果] 结合国医大师徐景藩的经验，陆师从气机升降、五行属性、精神情志方面阐述胆胃之间的关系，强调本病病因多端，虚实各有侧重，以肝胃不和、气机郁滞为发病之本。胆胃同病，多为实证、热证，多见胆病及胃，及肝胆失疏，气机不畅，郁而化火，不得宣泄，横逆犯胃，故以“通”和“降”为治疗大法，分肝胃不和、肝胃郁热、肝胃阴虚，寒湿中阻四型论治。验案为胆胃同病肝胃不和证，治拟疏肝和胃、理气止痛，以四逆散加减治疗，效果明显。[结论] 陆师分四型论治胆胃同病，临床经验丰富，疗效显著，颇有临床价值，以供同道参考。 |

| [Objective] To expound and summarize Professor LU Weimin‘s relevant academic thoughts， clinical ideas and experience in the treatment of gallbladder and stomach diseases based on syndrome differentiation of traditional Chinese medicine (TCM). [Methods] Through following clinical work， consulting ancient literature， summarizing medical record data， Professor LU’s experience in treating gallbladder and stomach diseases was discussed from the aspects of etiology， pathogenesis， prescription and medication. Additionally， a clinical case was proved. [Results] In combination with the experience of National Medical Master XU Jingfan， Professor LU elaborates the relationship between gallbladder and stomach from the aspects of Qi ascending and descending， five elements attributes， and psychic emotion. He stresses that the etiology of the disease is multi-faceted and the deficiency and excess are various focuses. Besides， the main causes of the disease are disharmony of the liver and stomach and Qi stagnation. The common syndromes of gallbladder and stomach disease are sthenia and heat， and most of them are gallbladder diseases affecting the stomach. Besides， disorder of the liver and gallbladder causes functional activities of Qi being not smooth which makes depression of Qi， turning into fire which is difficult to vent， and affects the stomach. Therefore，“smoothing” and“descending” are used to treat the gallbladder and stomach diseases which can be divided into four types (disharmony of the liver and stomach， stagnation of the liver and stomach heat， deficiency of the liver and stomach Yin， and obstruction of cold and dampness). The case was gallbladder and stomach disease with disharmony of the liver and stomach. The treatment was planned to soothe the liver and harmonize the stomach， regulate Qi and relieve pain， and modified Sini powder was adopted and the effect was obvious. [Conclusion] Professor LU’s treatment of four types of gallbladder and stomach diseases has sufficient theoretical basis of TCM and significant clinical effect， which is worthy of reference and promotion. |
