| [目的] 总结谢强教授运用转移兴奋灶针刺法治疗原发持续性耳鸣的经验。 [方法] 通过跟师学习及整理谢教授医案，查找相关文献，分析选穴及转移兴奋灶针刺法治疗耳鸣的机制，阐述转移兴奋灶刺法治疗原发持续性耳鸣的临床经验。 [结果] 谢教授在治疗时先刺远离患处的健侧远端腧穴，针刺时针尖朝患部方向强刺激，使脑部兴奋灶转移；后刺近端上部患处腧穴，轻手法、弱刺激以应答远端腧穴针气。针刺取穴选取健侧合谷、神门、外关等远端腧穴为主，重刺激；选取听会、翳风、翳明、百会等病位近端腧穴为辅，轻刺激作为应答。文中所举验案为肝肾阴虚、虚火上扰之耳鸣，谢教授运用转移兴奋灶针法，滋补肝肾、清养耳窍，疗效甚佳。 [结论] 谢教授重视“近病远治”，倡导“转移兴奋灶”针刺法治疗原发持续性耳鸣，疗效显著，且操作简便，适宜临床推广。 |

| [Objective] To summarize Professor XIE Qiang‘s experience in the treatment of primary persistent tinnitus by acupuncture with transferring excitable foci. [Methods] Through studying with the teacher and sorting out the medical records of Professor XIE， searching for relevant documents， this paper analyzed the mechanism of acupuncture for the treatment of tinnitus by selecting acupoints and transferring excitable foci， and expounded the clinical experience of the treatment of transferring excitable foci. [Results] Professor XIE’s treatment is mainly to acupuncture the distal acupoints on the healthy side far away from the affected area. When acupuncturing， the acupuncture point points upwards and strongly stimulates the main acupoints to transfer the excitement of the brain. Then acupuncture the proximal and upper acupoints on the affected side with light manipulation and weak stimulation to respond to the distal end. Acupuncture points are selected from the distal side of the healthy side， such as Hegu， Shenmen， Waiguan and other remote points as the main points， with heavy stimulation， and the proximal points at the affected side such as Tinghui， Yifeng， Yiming， Baihui， etc. are selected as supplements， and light stimulation acts as the response. The case was differentiated as deficiency of the liver and kidney Yin and disturbance of deficiency fire， and achieved good results after treated with nourishing the liver and kidney with acupuncture by transferring excitable foci. [Conclusion] Professor XIE Qiang attaches importance to “remote treatment of near diseases” and advocates the “transfer of excitement” acupuncture method for the treatment of primary persistent tinnitus， the curative effect is remarkable， which is easy to operate and suitable for promotion in clinic. |
