      [目的] 探讨益气消毒方对炎症性贫血(anemia of inflammation,AI)模型大鼠造血功能,以及白细胞介素-6(interleukin-6,IL-6)、铁调素(hepcidin)水平的影响。[方法] 40只Wistar大鼠中随机选取5只作为正常组,35只采用尾部皮下注射胶原乳剂的方法制备胶原诱导型关节炎(collagen-induced arthritis,CIA)大鼠模型,并在此基础上,连续7 d腹腔注射0.1 μg·mL-1的IL-6溶液0.4 mL,诱导胶原诱导型关节炎伴炎症性贫血(collagen-induced arthritis with anemia of inflammation,CIA-AI)。将造模成功的20只大鼠随机分成模型组,益气消毒方低、中、高剂量组,各组每天灌胃给予相应药物,连续2周。检测大鼠外周白细胞(white blood cell,WBC)、红细胞(red blood cell,RBC)、血红蛋白(hemoglobin,HGB)、平均红细胞体积(mean red blood cell volume,MCV)和血小板(platelet,PLT)水平;计算比较肝脏和单侧股骨指数;以酶联免疫吸附试验(enzyme-linked immunosorbent assay,ELISA)检测大鼠血清及肝组织匀浆IL-6、hepcidin、铁蛋白和铁水平;苏木素-伊红(hematoxylin-eosin,HE)染色观察骨髓病理形态学变化。[结果] 与正常组比较,模型组WBC、RBC和HGB水平明显降低,肝脏指数升高(P<0.05),单侧股骨指数显著下降(P<0.01);血清及肝组织匀浆IL-6、hepcidin、铁蛋白水平显著升高(P<0.01),铁水平明显下降(P<0.05);股骨骨髓中粒细胞和巨核细胞减少,髓腔内充填水肿的血管纤维脂肪组织。与模型组比较,益气消毒方高剂量组大鼠外周血WBC、RBC和HGB水平升高,单侧股骨指数升高,肝脏指数下降,血清及肝组织匀浆中铁水平显著升高(P<0.01),IL-6、hepcidin、铁蛋白水平下降(P<0.05);股骨骨髓病理形态改善。[结论] 益气消毒方可能通过调控机体炎症及骨髓造血关键因子IL-6、hepcidin水平,发挥纠正CIA-AI模型大鼠贫血的功效。

      [Objective] To investigate the effect of Yiqi Xiaodu prescription(YQXD) on the bone marrow hematopoietic function, interleukin-6(IL-6), and hepcidin in model rats with anemia of inflammation(AI). [Methods] Five of 40 Wistar rats were randomly selected as normal group, and 35 rats were injected subcutaneously with collagen emulsion in the tail to prepare the collagen-induced arthritis(CIA) rat model. The CIA-AI model rats were induced by intraperitoneal injection of 0.1 μg·mL-1 IL-6 solution 0.4 mL daily for one week. Twenty successful modeled rats were randomly divided into model group, YQXD low, medium and high-dose group. Each group was given corresponding drugs by gavage every day for 2 weeks. Peripheral white blood cell(WBC), red blood cell(RBC), hemoglobin(HGB), mean red blood cell volume(MCV), and platelet(PLT) were assayed. Hepatic and unilateral femur indexes were calculated and compared. The IL-6, hepcidin, ferritin, iron levels in serum and liver tissue homogenate were monitored by enzyme-linked immunosorbent assay(ELISA), and the pathological changes in bone marrow were observed by hematoxylin-eosin(HE) staining. [Results] Compared with normal group, the levels of WBC, RBC and HGB in model group were significantly decreased, and the liver index was increased(P<0.05), unilateral femur index was significantly declined(P<0.01); the IL-6, hepcidin, ferritin levels of serum and the liver tissue homogenate significantly increased(P<0.01), while iron level slid significantly(P<0.05); the granulocytes and megakaryocytes in femoral bone marrow reduced, and the medullary cavity was padded with edema vascular fibrous adipose tissue. Compared with model group, the WBC, RBC and HGB levels of peripheral blood, unilateral femur index, iron level of serum and liver tissue homogenate increased, while liver index decreased, and iron level of serum and liver tissue homogenate increased significantly in YQXD high-dose group(P<0.01), while the levels of IL-6, hepcidin and ferritin decreased(P<0.05); the pathological morphology of femoral bone marrow was repaired. [Conclusion] YQXD may play a positive role in improving anemia in CIA-AI model rats by regulating bone marrow hematopoietic function via IL-6 and hepcidin.
