| [目的] 探讨通络除痹法在糖尿病肾病中的应用，为临床治疗糖尿病肾病提供新的思路与方法。 [方法] 通过查阅多种古籍及现代文献，对络病理论进行概述。从络病理论出发，结合现代医学对糖尿病肾病的认识，对糖尿病肾病的络病病机进行理论探索，并以糖尿病肾病的病理学改变作为佐证。根据糖尿病肾病的络病病机，提出相应的治法，并探讨该治法在糖尿病肾病治疗中的具体应用。[结果] 基于络病理论，肾络痹阻为糖尿病肾病的络病病机，通络除痹法为其关键治法。根据虚实不同，通络除痹法分为消通法和补通法两种，消通法主要适用于肾络实痹证，可分为清热利湿、降浊解毒、活血化瘀、软坚消癥、搜风剔邪五种；补通法适宜于肾络虚痹证，可分为益气、补血、育阴、填精、温阳五类。[结论] 通络除痹法是治疗糖尿病肾病的关键治法，可为中医治疗糖尿病肾病提供新的思路与方法，值得临床参考与应用。 |

| [Objective] To explore the application of the method of dredging collaterals and removing obstruction in diabetic kidney disease(DKD)， and provide a new idea and method for clinical treatment of DKD. [Methods] Through consulting a variety of ancient books and modern literature， the theory of collateral diseases was summarized. Based on the theory of collateral disease， combined with the modern medical understanding of DKD， the pathogenesis of collateral disease of DKD was theoretically explored， and the pathological changes of DKD were taken as evidence. According to the collateral pathogenesis of DKD， the corresponding treatment was put forward， and the specific application of this treatment in treating DKD was explored. [Results] Based on the theory of collateral disease， the obstruction of kidney collaterals is the pathogenesis of collateral disease in DKD， and the method of dredging collaterals and removing obstruction is the key treatment method for DKD. According to the difference of deficiency and excess， the method of dredging collaterals and removing obstruction can be divided into two types: the method of dredging collaterals by eliminating evil and the method of dredging collaterals by tonifying deficiency and supporting healthy Qi. The method of dredging collaterals by eliminating evil is mainly suitable for the type of the obstruction of kidney collaterals caused by excessive evil， it can be divided into five types: clearing away heat and removing dampness， reducing turbidity and detoxification， promoting blood circulation and removing blood stasis， softening and eliminating hard tissue， and searching for wind and removing evil to dredge the collaterals. The method of dredging collaterals by tonifying deficiency and supporting healthy Qi is mainly suitable for the type of the obstruction of kidney collaterals caused by lack of healthy Qi， it can be divided into five categories: nourishing Qi， nourishing blood， nourishing Yin， filling essence and warming Yang to dredge the collaterals. [Conclusion] The method of dredging collaterals and removing obstruction is the key treatment method for DKD， which can provide a new idea and method for the treatment of DKD by traditional Chinese medicine， which is worthy of reference and application in clinic. |
