| [目的] 总结傅晓骏教授论治女性复发性尿路感染(recurrent urinary tract infection，RUTI)的经验。[方法] 通过随师侍诊，总结傅师论治RUTI的病因病机、辨证论治特点、临床诊疗思路及用药经验，并列举医案加以佐证。[结果] 傅师认为，女性RUTI病因病机复杂，属于中医学劳淋范畴，从肝脾肾三脏论述发病的机制与迁延难愈的原因，肝失疏泄、脾失健运或肾虚不固均可致劳淋。在临床辨治上，因劳淋初起多以实为主，后期多见虚实夹杂之证，傅师分期辨治，将劳淋分为急性发作期和慢性迁延期。急性发作期，治以清热毒、利湿浊，开枢机、清三焦。慢性迁延期，先辨脏腑，以肾虚为主，治以养先天、填肾精；以脾虚为主，治以补后天、益中气。傅师自拟方尿淋汤，在临床上运用甚广。所附医案，辨为劳淋急性发作期，治以清热解毒、利湿通淋，清利三焦、开郁利枢，方用尿淋汤加减，获满意疗效。[结论] 傅师分期论治女性RUTI，学术特色鲜明，为女性RUTI的中医治疗提供了新思路，其经验值得学习与借鉴。 |

| [Objective] To introduce and summarize the experience of Professor FU Xiaojun in treating female recurrent urinary tract infection(RUTI). [Methods] By following Professor FU‘s clinical work， the etiology and pathogenesis， syndrome differentiation and treatment characteristics， clinical diagnosis and treatment ideas and medication experience of Professor FU in treating RUTI were summarized， and a typical medical case was listed as evidence. [Results] Professor FU believes that the etiology and pathogenesis of female RUTI is complex， which belongs to the category of stranguria due to overstrain in traditional Chinese medicine. Professor FU discusses the pathogenesis of the disease and the reasons for the prolonged course and being refractory from the liver， spleen and kidney. It is expounded that the failure of the liver to disperse， the failure of the spleen and the insufficiency of the kidney can cause stranguria due to overstrain. In terms of clinical differentiation and treatment， due to the fact that excessive syndrome is more common at the beginning of the stranguria due to overstrain， and the late stage is often mixed with deficiency and excess， Professor FU divides the syndrome of stranguria due to overstrain into stages of acute attack phase and chronic progression phase. In terms of treatment method， in the acute attack phase， the treatment is to clear away heat and toxin， remove dampness and turbidity; open the cardinal organs to clear the tri-Jiao. In the chronic progression phase， the viscera should be identified first， if mainly for the kidney deficiency， the congenital nature should be nourished， and the kidney essence should be filled; mainly for spleen deficiency， the treatment should be replenishing the root， and benefiting the Qi of middle-Jiao. Professor FU’s self-prepared formula Niaolin Decoction is widely used in clinical practice. The attached medical case was identified as the acute attack phase of stranguria due to overstrain， and was treated by clearing heat and detoxifying， removing dampness and drenching stranguria， clearing and benefiting tri-Jiao， relieving depression and benefiting pivot， Niaolin Decoction was adopted and satisfactory results were achieved. [Conclusion] Professor FU treats female RUTI in stages， with distinctive academic characteristics， it provides a new idea for the traditional Chinese medicine treatment of female RUTI， and the experience is worth learning and reference. |
