| [目的] 分析《醉花窗医案》中对食积的诊治思想，提高临床辨治食积水平。 [方法] 通过对全书所载有关食积的医案13则进行分析，从鉴别诊断、运动疗法、遣方用药等方面总结王堉对食积的辨治特色与经验。[结果] 王堉常注意食积与他病的鉴别，以脉证结合为主要鉴别诊断方法，又以脉诊更为重要，描述食积之脉多为“右关坚而搏指”。王堉还善于运用运动疗法刺激中焦，振奋阳气，逐邪外出，治疗食积饮冷导致的急性腹痛，不药而愈。用药方面善用成方，以平胃散为主，灵活加减，未用泻下之药，却能达到泻下之效，常常使患者“解下秽物极多”而愈。[结论] 王堉诊治食积颇有特色，精于脉诊，善于鉴别食积与他病，善用运动疗法治疗食积所致急性腹痛，用药方面善用成方平胃散加减，值得后人借鉴学习。 |

| [Objective] To analyze the thought of diagnosis and treatment of food retention in Zuihuachuang Cases， so that to improve the level of diagnosis and treatment of food retention in clinical work. [Methods] By organizing the 13 cases of food retention in Zuihuachuang Cases， written by WANG Yu， the characteristic of diagnosis and treatment of food retention was summarized in the aspect of differential diagnosis， physical therapy and prescriptions. [Results] WANG Yu paid attention to the differentiation of food retention and other diseases by combining pulse diagnosis with syndrome， and the pulse of food retention was hard and forceful at Guan position on right side. WANG Yu was also good at using physical therapy in treating acute abdominal pain due to food retention with cold by stimulating middle-Jiao and invigorating Yang Qi to get rid of pathogenic factors. In the aspect of prescription， WANG Yu added or subtracted on the basis of Pingwei powder， and patients would recover after evacuating putrid and offensive smelling stool. [Conclusion] WANG Yu was good at distinguishing food retention and other diseases， and using physical therapy to treat acute abdominal pain. In the aspect of prescription， WANG Yu added or subtracted on the basis of Pingwei powder. |
