[目的] 探讨食物不耐受调控嗜酸性粒细胞(eosinophilia,EOS)脱颗粒在脾虚湿蕴型特应性皮炎(atopic dermatitis,AD)发病中的作用及机制。 [方法] 将2018年1月至2021年1月,广州中医药大学深圳医院(福田)皮肤科诊治的脾虚湿蕴型AD患者56例作为AD组,同时选入40例健康体检者作为对照组。测定两组血清中13种食物过敏原的特异性抗体免疫球蛋白G(immune globulin G,IgG),外周血EOS细胞上清液中EOS阳离子蛋白(eosinophil cationic protein,ECP)、EOS趋化因子(eotaxin)、EOS过氧化物酶(eosinophil peroxidase,EPO)和白三烯C4(leukotrienes C4,LTC4)水平,再进行比较分析。 [结果] 两组中排前4位的不耐受食物为鸡蛋(69.64% vs 32.50%)、牛奶(50.00% vs 20.00%)、鳕鱼(35.71% vs 10.00%)和大豆(25.00% vs 7.50%),且AD组这4类食物不耐受情况均分别明显高于对照组(P<0.05)。AD组食物不耐受程度较对照组严重,即在分级为3级时AD组食物不耐受率显著高于对照组(P<0.05)。AD组、IgG组的ECP、eotaxin、EPO和LTC4水平均高于对照组(P<0.01,P<0.001),且ECP、eotaxin、EPO、LTC4均与总IgG水平呈正相关关系(P<0.001)。[结论] 食物不耐受在脾虚湿蕴型AD患者发病中发挥重要作用,其机制可能是通过促进EOS脱颗粒诱发致病。 |

[Objective] To investigate the role and mechanism of food intolerance in regulating eosinophilia(EOS) degranulation in the pathogenesis of atopic dermatitis(AD) with spleen deficiency and dampness deposition syndrome. [Methods] From January 2018 to January 2021, 56 patients of AD with spleen deficiency and dampness deposition syndrome were enrolled as AD group, and 40 healthy subjects were enrolled as control group. The specific antibodies immune globulin G(IgG) against 13 food allergens in serum of the two groups and eosinophil cationic protein(ECP) in the supernatant of EOS cells in peripheral blood of each group were determined. The levels of ECP, eotaxin, eosinophil peroxidase(EPO) and leukotrienes C4(LTC4) were compared and analyzed. [Results] The top 4 intolerant foods in both group were egg(69.64% vs 32.50%), milk(50.00% vs 20.00%), cod(35.71% vs 10.00%) and soybean(25.00% vs 7.50%), and the intolerant levels of these 4 foods in AD group were significantly higher than those in control group respectively(P<0.05). The degree of food intolerance in AD group was more serious than that in control group, that was, at grade 3, the food intolerance rate in AD group was significantly higher than that in control group(P<0.05). The levels of ECP, eotaxin, EPO and LTC4 in AD and IgG groups were higher than those in control group(P<0.01,P<0.001). ECP, eotaxin, EPO and LTC4 were positively correlated with total IgG levels(P<0.001). [Conclusion] Food intolerance plays an important role in the pathogenesis of AD with spleen deficiency and dampness deposition syndrome, and the mechanism may be through promoting EOS degranulation to induce AD. |
