      [目的] 归纳总结王亚丽教授从“三因三态”辨治头痛的经验。[方法] 通过跟师临证,从病因病机、辨证论治两方面浅析王亚丽教授辨治头痛的思路,并附典型医案一则加以佐证。[结果] 王师认为,头痛病因病机的核心多为风、痰、瘀之邪单独或兼夹为患,上犯清窍致头部经、筋不利,络脉缠绊失常,血气滞而不通。病程日久则痰瘀互结,或形成“筋结”,常致头痛反复发作、缠绵难愈。王师临证谨守风、痰、瘀三因,紧扣经脉、经筋、络脉缠绊三态,治以祛风通滞为法,所选方药以祛风化痰、平肝疏肝者居多。若遇病程日久或反复难愈者,擅加搜风祛痰、逐瘀通络类药物;若“筋结”形成,则在辨证的基础上辅以针刺、按摩以解结舒筋、通经止痛。所举医案为风寒侵袭、枢机不利、日久成瘀所致头痛,以疏肝利胆、祛风止痛,兼活血通络为法,遣合方、用对药,临证收效显著。 [结论] 王师临证治疗头痛,谨守风、痰、瘀致经、筋、脉络缠绊三态失常之病因病机,辨证精准,见证施药,疗效确切,值得临床借鉴和推广。
英文摘要:
      [Objective] To summarize Professor WANG Yali‘s experience in treating headache from “Three Causes and Three States”. [Methods] Through following the teacher‘s clinical practice, this paper analyzes Professor WANG‘s idea of treating headache from etiology and pathogenesis, and differentiation and treatment, with a typical medical case as evidence. [Results] Professor WANG believes that the pathogenesis of headache is mainly caused by wind, phlegm and blood stasis alone or combined, which invades orifice leading to the obstruction of the head meridians and tendons, the disorder of collateral entanglement, and stagnation of blood and Qi. If the course of the disease lasts for a long time, phlegm and blood stasis may be intertwined, or have the formation of “tendon knots”, these factors are the reasons for repeated headaches and lingering refractory. Professor WANG is certified, abiding by the three causes of wind, phlegm and blood stasis, closely linked to three states of meridians, tendons and collateral entanglement. The treatment method is dispelling wind and dredging stagnation, and the selected prescriptions are mostly dispelling wind and resolving phlegm, calming and soothing the liver. If the course of the disease is long or“tendon knot” is formed, acupuncture and massage are supplemented on the basis of syndrome differentiation to relieve knots and relax tendons, relax channels and collateral to relieve pain. The medical case cited was the headache caused by wind and cold invasion, unfavorable cardinal, and stasis over time. The method was to soothe the liver and promote gallbladder function, dispel wind and relieve pain, combined with activating blood circulation and dredging collateral, combined formula and drug pair were used, and the clinical effect was remarkable. [Conclusion] Professor WANG‘s treatment of headache, adheres to the etiology of wind, phlegm and stasis leading to three states disorder of meridian, tendon and collateral entanglement, with accurate syndrome differentiation, usage of Chinese drugs according to syndrome, the efficacy is definite and worthy of clinical reference and promotion.
