| [目的] 探索挖掘衢州雷氏医学的学术特色。[方法] 采用传统医史文献整理研究的方法，从时病、内科、儿科、女科、老年病、杂病、针灸等方面对雷氏医学的学术特色进行系统分析、总结。[结果] 衢州作为“丹溪医派”“新安医学”“杨继洲针灸”之融汇处、涵养地，在诊治疾病方面独具特色。雷氏医学认为时医应正确掌握四时五运六气的变化规律，并构建了全新的以四时六气为纲目“知时论证、辨体立法”的时病辨治体系，立法处方提倡用法而不用方。内科广泛运用脏腑辨证，同时还结合六经辨证、三焦辨证、卫气营血辨证、八纲辨证及病因辨证，临证依据病邪特点而治。儿科强调四诊合参，首重望诊，临证当明辨体质差异，治疗因人而异；小儿脾胃易伤，治疗时重视小儿脾胃的特点，处处顾及脾胃。女科注意圆机活法，通权达变，善于抓住妇人经、带、胎、产的生理特点辨证用药。老年病大多本虚标实，治疗首先以扶正为主，祛邪为辅，并且扶正多从先、后天之本着眼。杂病需提纲挈领，以常见病因病机为纲，又注意区分常证变证、兼证夹证，临证不忘多法并用以提高临床疗效。针灸融合了新安针灸名家和杨继洲针灸的学术思想和理论，选穴注意辨病与辨证相结合。[结论] 衢州雷氏医学在传承中医精华的基础上创新了理论体系，充分挖掘了其中的价值内涵，对临床各科具有深入的研究及独到的见解，值得深入研究和积极推广。 |

| [Objective] To explore the academic characteristics of LEI‘s medicine in Quzhou. [Methods] The academic characteristics of LEI‘s medicine were systematically analyzed and summarized from the aspects of current diseases， internal medicine， pediatrics， gynecology， senile diseases， miscellaneous diseases，acupuncture and moxibustion by means of collating and studying the literature of traditional medical history. [Results] Quzhou， as the blending place and self-cultivation place of “Danxi medical school” “Xin‘an medicine” and “YANG Jizhou acupuncture and moxibustion”， has its own characteristics in diagnosis and treatment of diseases. According to LEI‘s medical theory， modern medicine should master the changing law of four seasons， five movements and six Qi， and construct a new system of disease differentiation and treatment with the objective of“Knowing the time and demonstrating， and treating according to body differentiation”， medication prescriptions advocates the usage of therapeutic methods rather than prescriptions. In internal medicine， syndrome differentiation of Zang-Fu organs is widely used， and it is combined with syndrome differentiation of six meridians， syndrome differentiation of tri-Jiao， syndrome differentiation of Wei-Qi-Ying-Xue， syndrome differentiation of eight principles and of etiology， treatment is made according to the characteristics of pathogenic factors. The pediatric department emphasizes the four diagnosis， visitation is the first important. The clinical syndrome should distinguish the difference of constitution， and the treatment varies from person to person； children‘s spleen and stomach is easily injured， the treatment should pay attention to the characteristics of children‘s spleen and stomach， taking into account the spleen and stomach everywhere. The gynecology department pays attention to adopting the different treatment methods according to the different pathogenesis conditions， and is good at grasping the physiological characteristics of women‘s meridians， belts， fetuses and parturients. Most of the diseases of the aged are of deficiency in origin and excess in superficiality. Firstly， the treatment should base on strengthening the vital energy and eliminating pathogenic factors， and strengthening the vital energy should start from the kidney and the spleen. Miscellaneous diseases need to be outlined， take the common etiology and pathogenesis as the key point， and pay attention to distinguishing the common syndrome and variant syndrome， the secondary and the combined syndrome， and remember to use various methods in clinical treatment to improve the clinical efficacy. Acupuncture combines the academic ideas and theories of Xin‘an and YANG Jizhou， and selection of acupoints should pay attention to the combination of disease and syndrome differentiation. [Conclusion] On the basis of inheriting the essence of Chinese medicine， Quzhou LEI‘s medicine innovates its theoretical system， fully excavates its value connotation， and has in-depth research and original views on various clinical departments， it is worthy of further study and promotion. |
