| [目的] 浅析王有奎教授论治慢性阻塞性肺疾病的临证经验。[方法] 通过跟师学习，精读王教授著作及临床相关医案，查阅相关文献，从中医“肺贵说”理论着手论病因病机，并从五脏五行具体辨治，结合顺四时养五脏，总结王教授运用中医药治疗本病的经验，并附验案一则加以佐证。[结果] 王教授认为，慢性阻塞性肺疾病为肺系疾病迁延愈久所致，肺体不用，肺气虚损乃本病之基。肺虚则病及五脏，脏腑气机失衡，生克制化失常，故从金土相爱、培土生金，金木交互、泻木清金，金火相持、复金益火，金水相生、生源益精四方面进行辨证论治，用药均为王教授自拟经验方。同时王教授十分注重顺应四时以养五脏，防患于未然。文中所附医案，王教授辨为肺肾两虚证，治以补肾纳气、益气健脾，方用补肾宣肺方。[结论] 王教授临证注重先查病之源，再查病之机，从五脏五行辨治慢性阻塞性肺疾病，临床疗效显著，经验可供参考。 |

| [Objective] To analyze the clinical experience of Professor WANG Youkui in the treatment of chronic obstructive pulmonary disease(COPD). [Methods] By studying from the teacher， intensive reading of Professor WANG’s works and clinical cases， and reading related documents， from the “lung expensive” theory it discusses the etiology and pathogenesis， specific differentiation and treatment from “Five Viscera and Five Elements”. At the same time， it summarizes the experience of Professor WANG in the treatment of this disease combined with the four seasons to nourish the five viscera， and one clinical record was attached for verification. [Results] Professor WANG believes that COPD is caused by the protracted lung disease， lung Qi deficiency is the basis of this disease. Lung deficiency damages five viscera， leading to the disordered Qi movement in viscera， abnormality of interpromotion and interrestraint. Therefore， differentiation and treatment should be carried out from four aspects: the metal and earth together， supplementing the spleen to nourish the lung; interaction of metal and wood， clearing liver fire and purging the lung; mutual support of metal and fire， invigorating the heart and nourishing the lung; intergrowth of metal and water， invigorating the lung and nourishing the kidney， and the recipes are made by Professor WANG. At the same time， Professor WANG focuses on adjusting five elements and five viscera in four seasons. The case was diagnosed as deficiency of lung and kidney by Professor WANG， and treated with tonifying the kidney and absorbing Qi， replenishing Qi and invigorate the spleen， the recipe used was Bushen Xuanfei Decoction. [Conclusion] Professor WANG clinically focuses on the first check the source of the disease， then grasped the pathogenesis， the clinical effect of treating COPD from the five viscera and five elements is significant， and the experience can be referred to. |
