| [目的] 总结郭立中教授从温运脾胃治疗疑难杂病的临证经验。[方法] 通过经典理论的探源，结合郭师学术经验相关文献，阐明温运脾胃的立法依据，归纳临证辨治要点及常见治法和用药规律，并附验案一则佐证。[结果] 从温运脾胃治疗疑难杂病，立法依据基于疑病多痰郁、难病多寒湿、久病多中虚的病证特点。临证辨治强调应动态把握疾病阶段，权衡温阳散寒力度，区别脏腑病位选药，权衡先后次第立法，常采用温通卫胃法、温降中土法、益火补土法治疗。附录胃积验案，从初期、中期、后期、末期分而治之，体现了先开表、再畅中、益火源、固根本的先后次第思维。[结论] 温运脾胃对疑难杂病辨治具有一定理论及现实指导意义，值得进一步探索。 |

| [Objective] To summarize Professor GUO Lizhong’s clinical experience in treating difficult and miscellaneous diseases by warming and smoothing the spleen and stomach. [Methods] By exploring classical theories and combining them with Professor GUO’s academic experience and relevant literature， this article clarifies the theoretical basis for warming and smoothing the spleen and stomach， summarizes the key points of clinical syndrome differentiation and treatment， common treatment methods， medication rules， and provides a verification case as evidence. [Results] The theoretical basis of treating difficult and miscellaneous diseases by warming and smoothing the spleen and stomach is established based on the syndrome characteristics of miscellaneous diseases with excessive phlegm and depression， more difficult diseases with cold and dampness， and more chronic diseases with middle-Jiao deficiency. In clinical syndrome differentiation and treatment， it is necessary to grasp the disease stage dynamically， weigh the efforts of warming Yang and dispersing cold， select the drugs in distinguishing the location of visceral disease， and distinguish the treatment sequence according to the condition， commonly using the methods of warming and opening Wei and stomach， warming and descending the functions of the spleen and stomach in the middle-Jiao， reinforcing fire and replenishing soil. The medical record of stomach accumulation adopted different treatment methods from the early stage， middle stage， late stage and end stage， and reflected the sequential thinking of opening the exterior first， regulating the ascending and descending functions of the spleen and stomach， and then increasing the kidney Yang， last fixing the foundation of the human body. [Conclusion] The method of treating difficult and miscellaneous diseases by warming and smoothing the spleen and stomach has significant theoretical and practical implications for the differentiation and treatment of difficult and miscellaneous diseases， and further exploration is recommended. |
