| [目的] 总结安效先教授运用加味金刚丸治疗儿科疑难杂病的临床经验和学术观点，为临床选方用药提供借鉴。[方法] 选取安师治疗发育迟滞、进行性肌营养不良验案2则，从中医病因病机、治则治法、方药详解和思路探析等方面，归纳总结安师治疗儿科疑难杂病的特色和经验。[结果] 加味金刚丸出自《医宗金鉴》一书，为治疗筋骨痿软症所制，具有滋肝肾、强筋骨、补气血、通经络的功效。安师指出，加味金刚丸适用于“五迟五软”“痿证”的患儿，在临证时重视肝脾肾三脏的作用，补益肝脾肾为本，补先天以培源，养后天以治本，润筋脉以调枢；同时活血通络、利湿祛邪，以使经脉通畅；标本同治，切中病机。安师根据不同症状进行加减用药时精雕细琢，既遵从中医经典理论，又充分结合现代药理研究成果，有效保证了临床疗效。同时也体现了安师在诊治疾病时善于抓主证，辨病机，不囿于西医病名而影响中医思路，充分体现了中医“异病同治”的诊疗思路。文中所举儿童发育迟滞、进行性肌营养不良均为儿科疑难杂症，经过加味金刚丸治疗后肌肉无力、行走困难的症状均得到了有效缓解。[结论] 安师治疗儿科疑难病时善于抓主证，辨证审因，治法明确，依法立方，用药精准，其经验值得学习和借鉴。 |

| [Objective] To summarize the clinical experience and academic point of view of Professor AN Xiaoxian in the treatment of difficult and complicated pediatric diseases with Jiawei Jingang Pill， so as to provide reference for the selection of drugs in clinic. [Methods] Two cases of Professor AN in the treatment of growth retardation and progressive muscular dystrophy were selected to summarize the characteristics and experience of Professor AN in the treatment of difficult and complicated diseases in pediatrics from the aspects of etiology， pathogenesis， treatment principles and treatment， detailed explanation of prescriptions and ideas， etc. [Results] Jiawei Jingang Pill comes from the book Yizong Jinjian. It is made for the treatment of impotence and softness of muscles and bones. It has the effects of nourishing the liver and kidney， strengthening muscles and bones， tonifying Qi and blood， and dredging channels and collaterals. Professor AN proposes that Jiawei Jingang Pill is suitable for children with “five late， five softness” and “impotence syndrome”. In clinical treatment， he attaches importance to the role of the liver， spleen and kidney， tonifying the liver， spleen and kidney， tonifying congenital source， nourishing nurture to cure root， moistening muscles and veins to regulate pivot; at the same time， promoting blood circulation and dredging collaterals， promoting dampness and dispelling evil to make meridians unobstructed; treating both manifestation and root cause， cut into the pathogenesis. Professor AN adds and subtracts drugs according to different symptoms， which not only following the classical theory of traditional Chinese medicine， but also fully combined with the results of modern pharmacological research， which effectively ensures the clinical effect. At the same time， it also reflects that Professor AN is good at grasping the main syndrome and distinguishing the pathogenesis in the diagnosis and treatment of diseases， which is not limited to the disease name of western medicine and affects the thinking of traditional Chinese medicine， which fully embodies the diagnosis and treatment idea of “treating different diseases with the same treatment”. The children with growth retardation and progressive muscular dystrophy mentioned in this paper were all difficult and complicated diseases in pediatrics， and the symptoms of muscle weakness and difficulty in walking had been effectively alleviated after being treated with Jiawei Jingang Pill. [Conclusion] Professor AN is good at grasping the main syndrome， syndrome differentiation， clear treatment， prescription according to law and accurate medication in the treatment of difficult pediatric diseases， and his experience is worth learning and using for reference. |
