| [目的] 总结林咸明教授针药结合治疗月经性偏头痛的诊疗思想及临床经验。[方法] 通过跟师临诊，收集典型病案，从“针灸止痛调神”“遣方调体固本”两方面，分析总结林师运用针灸结合经方辨治月经性偏头痛的诊疗思想与临床经验，并附医案一则加以验证。[结果] 林师认为，月经性偏头痛的病因病机主要是气血不通，阻滞经络；气血虚损，脑窍失养。林师强调治疗本病需要明分期、辨虚实、调体质、重病证，针灸与方药并举，将“镇痛”“调神”贯穿整个治疗过程，最终疼痛得止、神志得安、冲任调和、正气自得。文末所举验案为产后冲任失调、枢机不利所致之经行头痛，以针刺疏通经络、止痛调神，方药疏肝解郁、调补冲任，针药相合，终得良效。[结论] 林师治疗月经性偏头痛，紧扣不通、不荣之病机，针刺重视调神，选方结合经典，将针灸外治法和中药内服法相结合，拓宽了诊疗思路，其经验值得借鉴与推广。 |

| [Objective] To summarize the diagnosis and treatment ideas of Professor LIN Xianming in treating menstrual migraine with the combination of acupuncture and medicine. [Methods] Through learning from Professor LIN in outpatient service， collecting typical cases， from the two aspects of “acupuncture for easing pain and regulating the mind” and “dispatch prescription for adjusting and consolidating the body”， summarizing the treatment ideas and clinical experience of Professor LIN in treating menstrual migraine with acupuncture and medicine， proved by a medical case. [Results] According to Professor LIN， the pathogenesis of menstrual migraine is mainly the blockage of Qi and blood leads to the meridian block， and the deficiency of Qi and blood leads to the loss of brain blood. Professor LIN emphasizes that the treatment of this disease requires the identification of the stage， the identification of deficiency and excess， the emphasis on the syndrome， the regulation of physique， and the combination of acupuncture and medicine， with the use of “easing pain” and “regulating the mind” throughout the whole treatment process. Eventually， the pain relief， the mind settles down， the thoroughfare-conception vessels are harmonized and the vital Qi is obtained. The case attached was menstrual migraine caused by a postpartum imbalance of thoroughfare-conception vessels and unfavorable cardinal. Acupuncture was used to dredge meridians， relieve pain， and regulate the mind， and the prescription was used to soothe the liver， relieve depression， and adjust thoroughfare-conception vessels. The combination of acupuncture and medicine finally achieved good results. [Conclusion] In treating menstrual migraines， Professor LIN closely follows the pathological mechanism of obstruction and non-nutrition， attaches importance to the adjustment of mind in acupuncture， combines prescription selection with classics， combines the external treatment of acupuncture with medicine， and broadens the idea of diagnosis and treatment， and the experience is worthy of reference and promotion. |
